Dear Mr. Velan:
This is in response to your letters of May 23, 1986 and May 28, 1986 on the subject of NRC Information Notice (IN) 86-34.
We endeavor to make each information notice accurate and concise. When it appears that a significant error has been made, we are glad to issue a supplement.
We have re-examined IN 86-34 and from our perspective, the only substantive point of difference that we find is your position that 1270 ft.lb. is the appropriate bolt torque.
The information on the decision as to the appropriate bolt torque was obtained from the licensee, and is supported by the licensee's calculation that the stress imposed on the cast iron actuator housing by 1270 ft.lb. torque approached the specified strength of the cast iron.
Further support for the opinion that 1270 ft.lb. torque is excessive for the cast iron actuator housing is provided by the fact that, as you state, "large cone-shaped chunks had been pulled out of the actuator." The statement in the information notice is '!the failure... was the capscrews tearing out of the cast iron."
It would appear that Limitorque, recognizing the strength limit of the cast iron, specified an appropriate lower torque.
It was our understanding that Velan understood and agreed with the appropriateness of the reduced torque.
Subsequent to receipt of your letter, representatives of Gulf States Utility, and of our Region IV inspection staff, have confirmed that your field represen-tative and a field representative from Limitorque, at a meeting at the River Bend site in January, orally agreed that the 700 foot pound torque value was correct for this application.
l With respect to your question whethar short bolts might have been substituted when the actuator had earlier been reinstalled after rework by Limitorque, licensee representatives assure the NRC that the assembly bolts, when removed, i
were specifically " bagged and tagged," and used for reassembly of the actuator l
to the valve.
We note the cautionary assembly note on page 52 of the tear sheet from your l
manual.
This appears to relate to body-bonnet assembly. We did not have the benefit of the tearsheet (or manual) when we wrote the information notice, but our concern addressed assembly of the actuator to the yoke.

" rule" either encouraging or discouraging naming names of companies in informa-tion notices. Where identifying organizations appears to be helpful in under-standing the circumstances, we will do so. When product or supplier names are not pa.rticularly relevant to the circumstances they are not stated.
For your information the valve cited in the Shoreham event is a 4-inch Velan valve, the actuator is a Limitorque SMB-000.
In review of the information notice, other than our difference of opinion on an appropriate torque value for assembly capscrews, we see nothing that appears to criticize either Velan or Limitorque.
The information notice is oriented toward the owners, not the component suppliers.
I see no basis for issuing a supplement to the subject information notice.
