The following information is submitted in response to Inspection Report Nos. 50-321/86-15 and 50-366/86-15, dated June 30, 1986.
The report concerns the inspection performed by Messrs. P. Holmes-Ray and G.
M. Nejfelt from April 26 through May 3,1986.
Violation:
" Unit 2 Technical Specification (TS) 4.7.4.b, Amendment No. 51, which was in effect on August 5,
1985, requires that a snubber be declared inoperable if visible signs of leakage are present.
Contrary to the above, on February 26,
- 1986, hydraulic snubber 2E11-RHR-R90, was found leaking and not declared inoperable as required by TS 4.7.4.b, Amendment No. 51.
This condition was attributed to an inadequate snubber inspection and functional test procedure 52SV-SUV-001-2 which the licensee failed to revise following implementation of TS Amendment No.
51.
TS Amendment No.
51 was implemented by the licensee on September 4,1985, and changed the snubber visual inspection criteria.
Previous licensee corrective actions discussed in Licensee Event Report numbers 50-321/85-28 and 50-366/85-28 failed to prevent this type of recurrence.
This is a Severity Level IV Violation (Supplement I) - Unit 2 only."
RESPONSE TO VIOLATION:
Admission or denial of alleged violation:
This event, which had no actual or potential safety consequnces, did occur and was identified by Georgia Power Company.
The event was self reported as LER 366/86-006.

Reason for violation:
(As noted in the LER 366/86/006) i a
l "The cause of the event was an inadequate procedure.
An investigation determined that Technical Specifications knendment No. 51 I
i which was implemented on 09/04/85 included the more stringent visual inspection acceptance criteria (i.e., the leaking snubber could no longer be determined operable via a functional test).
At that time, procedure 52SV-SUV-001-2 should have been revised to include the new acceptance criteria.
However, the procedure was not revised due to a personnel error."
Corrective steps which were taken and the resul ts achieved:
Upon discovery of the snubber inoperability, a 72-hour LCO was initiated per l
Technical Specifications Section 3.7.4, ACTION Statement.
Immediate corrective action was accomplished on March 18, 1986, when the leaking (and thus, inoperable) hydraulic snubber 2E11-RHR-R90 was replaced and returned to service.
In addition, the records for hydraulic snubbers i
inspected following the site's implementation of Amendment 51 were reviewed.
It was determined that hydraulic snubber 2 Ell-RHR-R90 was the only leaking snubber.
An engineering evaluation was performed, as required by Unit 2 Technical Specifications Sections 4.7.4.c.
This evaluation concluded that the inoperable snubber did not adversely affect the corrponents it supported, because the snubber did not prevent the components from performing their design function.
As a result of this item, the
" Technical Specifications Surveillance Program" procedure (40AC-REG-001-0S) and the
" Revisions Licensing Documents" procedure 1
(43C-CPL-001-05) were revised.
Interim training relative to these i
changes was conducted for plant personnel on October 3,
1985, via Departmental Directive DGM 85-69.
No other incidents of this nature have occurred since the interim training was conducted.
Corrective steps which will be taken to prevent further violations:
The
" Hydraulic Shock and Sway Arrestor Inspection and Functional Test" procedure (52SV-SUV-001-2S) will be revised to reflect the requirements of the current Unit 2 Technical Specifications prior to the next scheduled performance of the procedure.

Date when full compliance was achieved:
Compliance was achieved on March 18, 1986, when the leaking snubber was replaced, and the engineering evaluation was performed, as required by the Technical Specifications.
