NRC 40500 TEAM INSPECTION 50-336/99-01
Dear Mr. Bowling:

This letter transmits the results of the NRC team inspection at the Northeast Utilities (NU)
Millstone Unit 2 facility involving the review of the implementation of the corrective action
- program. This team inspection was performed onsite from January 19-22 and February 1-5,
1999, using NRC Inspection Procedure 40500, " Effectiveness of Licensee Controls in
identifying, Resolving, and Preventing Problems." The results were verbally communicated to
you and your staff at a public meeting held on February 17,1999, at the Millstone Training
Center.
Overall, the team found that the Unit 2 staff was effective in identifying, resolving, and preventing -
equipment problems and personnel performance issues. The problem identification threshold
was observed to be low, with a high volume of items being generated and processed on a daily
basis. The team's review of the root cause evaluations for significant problems founel them to be
generally thorough and the corrective actions appropriate. Corrective action assignments and
prioritizing were observed to be proper and well tracked. A review of the departmental self-
assessments found the reports to be generally self-critical and constructive. The team found the
Nuclear Oversight organization to be quite active and involved with the day-to-day operation of

the unit, as well as, the independent oversight and safety committees. A sampling of Employee

Concems Program cases found the issues to have been appropriately processed through the
program.

dealing with the items examined by the team at Unit 2. However, your ability to schedule and
/
complete work is still maturing, and when considered together with the work associated with the
Unit 2 restart and Unit 3 refueling outage, represent a significant continuing challenge.
Successfully addressing the current, large backlog of issues and emergent work arising from
both units, will require continued, close management attention to ensure appropriate
prioritization and timely completion of corrective actions.
L Based on the results of this inspection, the NRC has determined that one violation of NRC
requirements occurred. This violation is being treated as Non-Cited Violation (NCV), consistent
with Appendix C of the Enforcement Policy. The NCV is described in the subject inspection

report. If you contest the violation or severity level of the NCV, you should provide a response
