NOTICE OF VIOLATION (NRC INSPECTION REPORT NO. 70-1257/86-06)

This letter refers to the inspection conducted on July 28-30,,1986, at'the Horn _

Rapids Road facility, Richland, Washington.

The violation was identified by NRC Physical Security Inspector, A. D. McQueen, and discussio~ns of the inspection findings were held by the inspector with yourself and members of the Exxon Nuclear Company staff on July 30, 1986.

'On July 28, 1986, it was found that a sector of perimeter barrier lighting was inoperative.

The root cause of the violation appears to be a failure to appropriately report the inoperative lighting and follow-up on it's repair.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violation described I.

in the enclosed notice has been classified at a Severity Level IV.

You are required to respond to this letter and should follow the instructions 4

specified in the enclosed Notice when preparing your response.

In your response, you should document the specific actions taken and any additionale r

actions you plan to prevent recurrence.

After reviewing your response to thii.i Notice, including your proposed corrective actions, the NRC will determine whether further NRC enforcement action is necessary to ensure compli 2nce with NRC regulatory requirements.

InaccordancewithSection2.790(d)oftheNRC's"RulesofPractice,"Part5, Title 10, Code of Federal Regulations, documentation of the findings 'of your safeguards and security measures are exempt from public disclosure; therefore, the enclosed inspection report will not be placed in the Public Document Room and will receive limited distribution.
