Purpose. 'Ib implement a quality assurance program for packaging and transporting radioactive material associated with industrial radiography required in reference (a).

2.

Scoce. The quality assurance program outlined in this instruction applies to packaging and transporting radioactive materials listed in reference (c) only.

3.

Responsibility The Radiaticn Safety Officer is responsible for establishing and managing a.

the quality assurance program. Specifically he will:

(1) Ensure all perscnnel involved in the program receive initial / periodic refresher training and hv - nt the training in the individual's training records.

(2) Review the quality assurance program periodically and make changes as required.

(3) Ensure all defined quality assurance procedures, engineering procedures and specific provisions of the package design certification are satisfied.

b.

The Senior Radiographer is responsible for performing quality assurance operations under the guidance of the Radiation Safety Officer.

4.

Organization a.

Organizaticnal Structure. The chain of cmmand for the radiography program is as follows:

(1) Ccmnanding Officer (2) Executive Officer (3) Repair Officer (4) Radiation Safety Officer

The Chuanding Officer is the licensee for this activity and has overall responsibility foc matters concerning the licensed materials. He shall designate the Radiation Safety Officer in writing.

The Radiation Safety Officer is responsible to the Ccrmianding Officer for c.

matters relating to the quality assurance program. The Radiation Safety Officer shall be gaalified in accordance with reference (b). He directly supervises the Senior Radicgrapher in the performance of his quality assurance duties.

d.

The Senior Radiographer is responsible to the Radiation Safety Officer for coordinating receipts, handling, transfers, tests and inspecticms of radioactive material ocmducted in accordance with reference (b). The Senior Radiographer is to be qualified in accordance with reference (b).

5.

Document Control The Senior Radiographer is responsible for th-nt control. Specifically a.

he will:

(1) Maintain all certificates of empliance for shipping packages used by this activity cn file with other records required by this instruction.

(2) Enter any changes to certificates of ccrupliance upon receipt.

6.

Receiving, Handling, Storage and Shimnt Receiving, handling, storage and shipnent of radioactive materials shall be a.

performed in accordance with reference (b).

7.

Inspection, Tests and Operating Status Inspection, tests and operating status of shipping packages shall be a.

performed in accordance with reference (b).

8.

Control of Measuring and Test Equipment Control of measuring and test equignent shall be in accordance with a.

9.

Quality Assurance Records

'Ihe following records shall be maintained by the Radiological Ccntrols a.

Officer:

(1) All leak test and swipe tests performed by this activity.

(2) Shipping <hments received frcxn the shipper.

ACADIAIMST 9000.4 (3) A copy of the Government Bill of Lading, signed by the carrier's representative and any other pertinent hwnts relative to the shipnent of the shipping container.

(4) Copies of all audit reports and any correspondence dealing with audits of the quality assurance program.

Records shall be maintained in accordance with reference (b).

c.

All quality assurance records shall be identified and retrievable. Except as specified in reference (b), all records will be nuintained onboard for a period of 2 years.

10. Audits The industrial radiography packaging and transporting quality assurance a.

program will be audited at least once each year. The Radiation Safety Officer shall schedule the audit.

b.

The Ccmnanding Officer shall designate the auditors in writing.

Audits shall be performed by persons not directly responsible to the c.

Radiaticn Safety Officer.

d.

Areas to be covered in the audit shall include:

(1) Responsibilities and records of the Radiation Safety Officer.

(2) Responsibilities and records of the Senior Ra3iographer.

(3) Training records (4) General record keeping (5) Cmpliance with quality assurance program requirements (6) Previous audit reports cn file, and cbcumentation of necessary corrective action.

(7) Radiographic equipment /shi@ing cxmtainers, certificates of cmpliance for containers used and letters of authorization frcru the Nuclear Regulatory Ocuraissicn to use the containers.

(8) Receipt and transfer h = ants.

(9) Equipnent for conducting swipe / leak tests.

(10) Swipe and leak test results.

The auditor shall report his audit results to the Ccmnanding Of ficer in e.

writing, providing a copy to the Radiation Safety Officer.

f.

The Radiation Safety Officer shall reply in writing to the Comunding Officer, providing a copy to the auditor, concerning each audit deficiency.

g.

The Radiation Safety Officer will nuintain a copy of all audit reports including corrective action taken.
