Response to Inspection Report 86-12 This letter is written in response to Reference b), which indicates that one of our activities was not conducted in full compliance with Nuclear Regulatory Commission requirements.

This Level V violation was identified as a result of an inspection conducted by your Mr. K. Manoly during the period May 12-16, 1986.

Information is submitted as follows in answer to the alleged violation con-tained in the Appendix to your letter.

ITEM Vermont Yankee Technical Specification 6.5.A.5 requires detailed writ-ten procedures be adhered to for maintenance (modification) operations which could have an effect on the safety of the reactor. Yankee Atomic Electric Company Operational Quality Assurance Program YOQAP-1-A and Morrison Knudsen Quality Assurance Program reiterate the same requirements regarding the accomplishing of activities affecting quality through documented procedures. Morrison Knudsen Field Quality i

Procedure FQP-9.1 requires, in Section 4.9, technicians to complete a Visual Weld Examination Report for work examined.

Contrary to the above, visual weld examination on recirculation piping l

whip restraints No. R6A and R98 were not documented on a Visual Weld l

Examination Report as required.

RESPONSE

Welds on recirculation piping whip restraints No. R6A and R9B were inspected and accepted by contractor Quality Control person-nel on April 24, 1986 and May 4, 1986 respectively, as required by Morrison Knudsen Field Quality Procedure FQP-9.1.

These inspections were documer..ed on Weld History Cards by inspector initials, date, certification type and level, step number, and accept / reject criteria. However, contrary to Morrison Knudsen Field Quality Procedure FQP-9.1, the inspector failed to dupli-cate this information on a Visual Weld Examination Report.

These discrepancies were resolved by the Contractor QC/ Licensee QA prog rams.

Morrison Knudsen Nonconformance Report No. 237, and Vermont Yankee Nonconformance Report No. 86-82, were generated on May 20, 1986 and May 22, 1986, respectively.

It was determined that the discrepancies were acceptable as-is based on the inspections per-formed on Apr:1 24 and May 4 since the appropriate accept / reject criteria were 1ound on the Weld History Card.

In order to pre-vent recurrencs, all Morrison Knudsen inspectors were reinstructed to complete a Visual Weld Examination Card for all applicable inspections.

We consider the above actions adequate to prevent recurrence of these events and to achieve full compliance; however, should you have any questions or desire additional information, please contact us.
