4 RESPONSE TO NOTICE OF VIOLATION VIO 88-10-02 Violation:
On September 9, 1988, an individual was included on the Authorized User List who had not satisfactorily completed the annual retraining in that he had not received a passing grade on the written examination administered on April 26, 1988.

Admission or denial of the violation:
I admit to the violation.

The reason for the violation.
l The person responsible for issuing the Authorized User List in-1 cluded the name of the individual who did not pass the written l
examination because he used the list of attendees at the annual retraining rather than the training record files.

The corrective steps which have been taken and the results achieved:
1 A review was performed of the training records to determine thst all individuals whose names that appeared on the Authorized User List had achieved a passing grade on the written examination. The i
list is now correct.

The corrective steps which will be taken to avoid further viola-tions:
1 T,1e responsibility for maintenance of the Authorized User List has been reassigned to the Record Center Clerk.
This individual has been instructed to review the list af ter each training period and to revise the If st based on the passing grades entered in the training records.

The date when full compliance will be achieved:
I feel that we now fully comply with the requirement.
