This Supplement documents the completion of the review of Corrective Actions resulting from Significance Level 3 Discrepancy Reports as discussed in Section 6.3 of the Final Report.

Corrective Actions Review - Level 3 Discrepancy Reports At the time of the issuance of the Millstone Unit 2 Independent Corrective Action Verification Program Final Report - Volumes I and II (NUM2-PPNR-2248-L, dated January 12, 1999),
NNECo had not completed the corrective actions associated with the Significance level 3 Discrepancy Reports generated by Parsons. The engineering activitics associated with these corrective actions have now been completed by NNECo and Parsons has completed the review of a selected sample of these corrective actions. The purpose of this supplement to the Final Report (Volume II - Section 6.3) is to document the results of this review.

Level 3 Discrepancy Report Corrective Action Sample
- Results The purpose of the Significance Level 3 Discrepancy Report Corrective Action review was to assure that NNECo has taken the appropriate steps to correct the Significance Level 3 (Licensing i
and Design Basis) issue that was identified in the DR. Closure packages were assembled by NNECo for Parsons review for each of the selected Discrepancy Reports.

The review concluded that adequate corrective actions were developed for the licensing and design basis issues by NNECo for twenty-nine (29) of the thirty (30) closure packages. In one DR closure package, NNECo did not include vent and drain valves, which were added as part of the l
modification, to the appropriate leakage testing procedure. This was a minor completeness issue and a Condition Report was generated by NNECo to track and correct this item. All other technical aspects of this DR were reviewed with satisfactory results.
1.3.
Level 3 Discrepancy Report Corrective Action Review
- Conclusions Based upon the results of the Significance Level 3 Discrepancy Report Corrective Action resiew, the NNECo Corrective Action Process is effective in correcting the Licensing and Design Basis issues.
1.4.
Overall Conclusions Based upon the lack ofidentification of any significant Licensing and l}esign Basis issues in any of the three scopes of Corrective Action review, it is concluded that the NNECo Corrective Action Process was effective in identifying and correcting Licensing and Design Basis issues. Therefore, the Corrective Action review conclusions presented in Section 1.4.4 of Volume I remain unchanged.
Section 1.5 of Volume f (Overa!! Conclusions) remain valid.
