REVISED FORM 591 Bf/ memorandum dated. April 19, 1982, Mr. Higginbotham sent to the Regional Fuel Facilities and Material Branches a revised Form 591. Mr. Higginbotham recommended ihat it be used for inspections in which "Draf t inspection Reports" are prepared as provided in IE MC 1005-40. We have received several phone calls regarding the i
use of this fom. Af ter consultation with Mr. Higginbotham, we offer the following responses to these questions.
1.
Form 591 use is restricted to Priorties !! through VII material licensees.
2.
Fcrm 591 is a notice of violation and not a substitute for inspec. tion i
reports or inspector notes.
3.
Fom 591 use should be as the notice of violation on inspections in which the violations are at a Severity level IV or below provided an rinforcement conference or escalated enforcement is not expected to be associated with the violations.
If an enforcement conference or escalated enfrarcement is expected, the traditional 10 CFR 2.201 notice of violation should be used i
which requires the licensee to fonnally respond to the notice of violation in order to lay a clear basis for the future escalated actior.
