In order that Equitable Gas Coupany may centinue to ship and

receive radioactive materials in excess of 20 curies, it is requested that renewal of Quality Assurance Program Approval No. 0469 be granted.
Please find enclosed a copy of the Quality Assurance Program, prepared in accordance with 10 CFR Part 150.20 and satisfying the provisions of 10 CFR Part 71.
I have enclosed a check in the amount of $150.00 which satisfies the required renewal fee.
