SUBJECT: NRC INSPECTION REPORT NO. 50-498/99-02; 50-499/99-02

Dear Mr. Cottle:

This refers to the inspection conducted on January 10 through February 20,1999, at the South

Texas Project Electric Generating Station, Units 1 and 2, facility. The enclosed report presents

the results of this inspection.

We are concerned about the operator errors recently noted at your facility. During this

inspection period, troubleshooting / corrective maintenance activities being performed by

operators caused a plant trip and an uncontrolled reactivity increase. In both cases, existing

barriers were insufficient to preclude occurrence of these events. The plant trip involved

excersive reliance upon the skill of an individual rather than on an appropriately thought out

plan that was properly reviewed and supported by adequate procedural guidance. As was

noted in your problematic response to the June 1998 switchboard fire, electrical theory

knowledge deficiencies also contributed. The reactivity excursion associated with the moisture

separator work also demonstrated poor planning, an inadequate use of available indications,

and operator knowledge deficiencies.

In both cases, South Texas Project conducted an intrusive review of the circumstances leading

to the event and identified corrective actions which should help preclude similar recurrences.

However, it should be noted that knowledge deficiencies in the application of electrical theory

were recognized as a contributable factor to the September 1998 plant trip, and this deficiency I

has not been adequately ddressed.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosure will be placed in the NRC Public Document Room (PDR).
