ATTACHMENT 1 10 CFR 2.201 INFORMATION PUBLIC SERVICE ELECTRIC AND GAS COMPANY HOPE CREEK GENERATING STATION RESPONSE TO NOTICE OF VIOLATION INSPECTION REPORT NUMBER 50-354/88-22 As described in Appendix A of your September 16, 1988 letters

- 10 CFR 50, Appendix B, criterion XIV specifies that measures shall be established for indicating the operating status of components of a nuclear power plant, such as safety tagging valves and switches, to prevent inadvertent operation, and

- Contrary to the above, the measures established for safety tagging valves were not adequate, in that the relay for valve GS-HV-4958 was erroneously removed on May 27, 1988, rendering the valve inoperable: and the valve was not restored to operability on May 30, 1983, when the deficient system tagging was removed.

1. PUBLIC SERVICE ELECTRIC AND GAS COMPANY DOES NOT DISPUTE THE VIOLATION.

j l 2. THE ROOT CAhSE OF THE VIOLATION WAS PERSONNEL ERROR:

Wherein an equipment operator and shift technician, safety during a tagging operation, removed the relay for

' valve GS-HV-4958 instead of the requested relay for valve GS-HV-4952, thereby making the wrong valve inoperable.

Upon completion of the work that required the safety tagging, valve GS-HV-4952 was stroke tested satisfactorily (its relay having never been removed):

however, the improperly removed relay...which made valve

, GS-HV-4958 inoperable...was not reinstalled until the error was discovered at a later date.

3. IMMEDIATE CORRECTIVE ACTIONS:

Those personnel involved in the improper equipment safety tagging were counselled regarding the incident and their individual responsibilities. This instruction was reinforced by conducting training for all equipment operators and shift technicians on panel locations, configurations and labelling 4

schemes.
