- 1. Describe the problem areas addressed in the report. Prior to undertaking to address those areas through sampling, what did Applicants do to define the problem areas further? How did it believe the problems arose? Whrit did it discover about the QA/QC documentation for those areas?
How extensive did it believe the problems were?
Responsa:
The original issue raised by the NRC-TRT involved two isolated cases of separation violations in the control room control boards involving panel-mounted devices. During the inspections of the control room control boards and vertical ventilation panels !

t.. 1 performed during the implementation of Action Plans I.b.1 and I.b.2, additional violations involving panel-mounted devices were noted. As a-result, the scope of this action plan was expanded to include all separation violations identified in electrical panels that did not involve Servicair flex, which are addressed in Action i Plans I.b.1 and I.b.2.
The CPRT third-party did not determine a priori that QA/QC activities or any other activities were t
responsible for the identified deviations. .This investigation of root causes was performed after all pertinent information regarding the deviations had been collected. The generic implication evaluation of the root causes to determine the extent to which other panels could be similarly affected was subsequently ,
performed. Based on this evaluation, it was determined i that the root causes presented in the Result's Report would apply to all electrical panels requiring separation between redundant cables. Therefore, the
- corrective action to be taken involves the reinspection of all panels which must meet the separation criteria.

- 2. Provide any procedures or other internal documents that are necessary to understand how the checklists should be interpreted or applied.
Response
As noted above, the inspection report used by the CPRT third-party inspectors during their inspection of the control boards and vertical ventilation panels was a part of Quality Instruction QI-004. All revisions of this procedure are included in File I.b.1-3 of the ISAP I.b.1 Working File. In addition, all background information used to develop the criteria in the procedure are included in this file.

Response
The Unit 1 control boards and vertical ventilation panels were inspected shortly before the CPRT third-party inspection. During the Project inspection, the procedure used was QI-QP-11.3-40, Revisions 19, 20 and 21, " Post Construction Inspection of Electrical Equipment and Raceways." In addition, in-process inspections had been previously performed in l

accordance with QI-QP-11.3-28, " Class 1E Cable Terminations."
As noted in the Results Report, preliminary, conservative inspection guidelines were used during the l CPRT inspection. This was done so as to provide an indication of the extent of potential modifications required for compliance with the final CPRT separation criteria. In addition, the information obtained from this inspection was used in the testing and circuit evaluation performed to demonstrate the adequacy of Servicair flex as a barrier.
The separation distances specified in the CPRT inspection guidelines were greater than or equal to the distances specified in the criteria in QI-QP-11.3-40 and QI-QP-11.3-28.

- 4. Explain the extent to which the checklists contain fewer attributes than are required for conformance to codes to which Applicants are committed to conform.
Response
With one exception, the CPRT Inspection Report included a check of all physical separation attributes required by IEEE 384-1974, "IEEE Trial-Use Standard

Criteria for Separation of Class lE Equipment and Circuits," and Regulatory Guide 1.75, Revision 1,
" Physical Independence of Electric Systems." The one exception is that the separation of cables, each 4
enclosed in a vendor wireway, was not verified. Since the Project criteria at that time allowed two redundant cables in metallic conduits to touch, the metallic wireways used in the control boards and vertical h ventilation panels are allowed to be in contact with '
one another. Therefore, no inspection of separation between vendor wireways was required.

- 5. (Answer question 5 only if the answer to question 4 is that the checklists do contain fewer attributes.) Explain the engineering basis, if any, for believing that the safety margin for components (and the plant) has not been degraded by using checklists that contain fewer attributes than are required for conformance to codes.

Response
The following revisions were made to the CPRT inspection procedure QI-004:
Initial issue 1/18/85 Revision 1 2/18/85 Revision 2 2/26/86 Revision 3 3/6/85 Revision 4 3/12/85 Revision 4 of QI-004 wee uced for all control boards and vertical ventilation panels except control boards CP1-ECPRCB-01 thru -04, which were inspected to Revision 2 and control boards CP1-ECPRCB-05 and -07, which were inspected to Revision 3. Since the first revision used in an actual inspection was Revision 2, changes made prior to Revision 2 do not affect the inspections performed. In addition, changes made subsequent to Revision 2 of the procedure do not affect the panels inspected to revisions earlier than Revision 4.
- 1. Revision 3 changed the inspection method for missing barriers. Rather than perform a paper trace to determine if documentation exists for the

installation / reinstallation of missing barriers, the inspector was to determine whether or not the barrier is required for separation purposes. In addition, changes were made in the way in which the Inspection Reports were to be-completed for control boards CP1-ECPRCB-10 and -11 and the vertical ventilation panels (CPX-ECPRCV-Oi and--03).
- 2. Revision 4 provided additional clarification on the inspection criteria for cont'rol boards CP1-ECPRCB-10 and -11
, and the vertical ventilation panels (CPX-ECPRCV-01 and -03).

- 7. Set forth the duration of training in the use of checklists and a summary of the content of that' training, including field training or other practical training. If the training has changed or retraining occurred, explain the reason for
the changes or retraining and set forth changes in duration or content.
Response
Classroom and field training were conducted for Revision 0 of QI-004. A 50-minute classroom session and a one-hour field course were held for inspectors.
Training was performed for Revision 2 of QI-004, the first revision actually used in the inspection of the
- panels. Revisions 3 and 4 contained minor changes that were addressed in formal discussions with the inspectors.

- 8. Provide any information in Applicants' possession concerning the-accuracy of use of the checklists (or the inter-observer reliability in using the checklists). Were there any time periods in which checklists were used with questionable training or QA/QC supervision? If applicable, are problems of inter-observer reliability addressed statistically?
Response
There is no information available concerning the accuracy of the use of the checklists or inter-observer reliability in using the checklists. The subject inspections were conducted primarily in the first quarter of 1985; there were no time periods in which tie checklists were used with questionable training or

QA/QC supervision. All inspectors were certified Level II or above in accordance with the requirements of ANSI N45.2.6-1978 and Regulatory Guide 1.58, Revision 1.

- 9. Summarize all audits or supervisory reviews (including reviews by employees or consultants) of training or of use of the checklists. Provide the factual basis for believing that the audit and review activity was adequate and that each concern of the audit and review teams has been resolved in a way that is consistent with the validity of conclusions.
Response
No ERC Corporate .3udits or Supervisory Reviews were conducted which were directly related to the use of the checklists or training applicable to this action plan. However, ERC Corporate Audits 85-01 and 85-03 and QA Surveillances I85-01 and III85-09 verified that ERC inspectors were qualified and certified in accordance with ANSI N45.2.6-1978 and Regulatory Guide 1.58, Revision 1. In addition, applic'able ERC inspectors received classroom instruction and on-the-job training in the requirements of QI-004, which is the instruction which governed the reinspection effort.

- 10. Report any instances in which draft reports were modified in an important substantive way as.the result of. management action. Be sure to explain 1 any change that was objected to (including by;an i employee, supervisor or consultant) in writing or in a meeting in which at least one supervisory or management official or NRC employee was present.
Explain what the earlier drafts said and why they were modified. Explain how dissenting views were resolved.

. substantive changes during the generation, review, and editing of the report.resulting from management action.
Accordingly, our answer to this question is: None.

- 11. Set forth any unexpected difficulties that were encountered in completing the work of each task force and that would be helpful to the Board in understanding the. process by which conclusions were reached. How were each of these unexpected difficulties resolved?
Response
Several unexpected difficulties were encountered during the implementation of this action plan that resulted in. delaying the completion of this action plan. These difficulties involved the identification
z.
of the root causes for the identified separation
' violations and the interface with the Project.
When a separation violation was identified, it was difficult to determine when the violation first i
existed. The violation could have been a result of improper installation followed by inadequate inspection. On the other hand, the original installation could have been performed adequately, but later activities could have resulted in.the violation.
In determining the root causes of the violations, the specific identification of the source of the problem would have facilitated the subsequent evaluation. In addition, several different groups had authorization to perform work in'the electrical panels, thereby further complicating the issue as to who could have caused a separation violation after initial installation. In lieu of facts as to when separation violations occurred, the root cause analysis performed for this 5
action plan assumed that separation violations existed
, both due to inadequate initial installation and also due to subsequent work activities.
The other area of difficulty involved the interface between the CPRT and the Project. The
interface was strained at times which resulted in an inefficient use of resources. Disagreements on the need to perform certain activities, such as the need for the CPRT third-party to review Operations procedures, resulted in the delay of completion of the action plan.
The lack of emphasis on the completion of the CPRT action plan shown by the Project resulted in the CPRT third-party spending excessive time to close out the issues, especially where the Project's input was required.
However, none of the above items had an effect on the activities or results of this ISAP.

Response
Open items are addressed in Section 7.0, " Ongoing Activities," of the Results Report. The results of these activities will be reported in a supplement to the Results Report. Any potential ambiguities are addressed in the response to Question 14.

To the best of our knowledge, no conflicts of interest exist.

- 14. Examine the report to see that it adequately discloses the thinking and analysis used. If the language is ambiguous or the discussion gives rise to obvious questions, resolve the ambiguities and anticipate and resolve the questions.
Response
There is one potential area in the Results Report that could be confusing to someone not familiar with the subject of the Action Plan -- the basis for the different types of inspections performed.
During the implementation of the I.b.4 Action Plan, several different physical reviews of electrical panels were performed; some of these reviews utilized different criteria. In addition, previous or future l
inspections of the panels were referenced in the Results Report. The following summary of inspection t
activities and the criteria used should provide a clearer understanding of the physical reviews performed:
Activity 1: . Post construction inspection of the Unit 1 control boards and vertical ventilation panels.
Performed by: TUGCO inspectors Date: November 1984 - January 1985 Criteria used: The " original" Project criteria in existence since November 1980.
Activity 2: CPRT third-party inspection of the Unit 1 control boards and vertical ventilation panels.
Performed by: CPRT third-party inspectors Date: February - April 1985 Criteria used: Preliminary, conservative separation guidelines designed to envelope the
" final" CPRT criteria to be developed as a result of this action plan.
Activity 3: TUGCO inspection of the Unit 1 control boards and vertical ventilation panels to validate the CPRT findings and to provide a Project inspection to the " final" CPRT criteria.

2.2 The scope of this procedure is the cabling and wiring within selected control panels and cabinets as identified in the above references. The scope of inspections is any interaction between safety related wiring and components, excluding vendor wireway to vendor wireway (per Ref.
l 1.5).

. . Revision: 4 3.0 RESPONSIBILITIES 3.1 The third party inspectors (ERC) shall be responsible for conducting inspections and for completion of inspection reports. The Electrical / Instrumentation Review Team Leader is responsible for the review and approval of the inspection reports.
4.0 INSTRUCTION 4.1 The. Quality Inspector shall inspect the specific Main Control boards and vertical panels (Ref.1.1) to verify that:
' ACTIVITY 1 -Internal lighting and 120 VAC convenience receptacle wiring not routed in rigid conduit EMT, or Unistrut P-1000 channel is separated at least 6 inches'from safety related wiring and components (Ref. 1.1, para._3 and Ref. 1.5).
ACTIVITY 2 -Flexible conduit attached to control switch modules does not exceed 3 feet in length without additional support (Ref.
1.1, para. 2 and Ref. 1.3).
ACTIVITY 3 -Verify that wiring is separated in accordance with, Ref 1.1, para. 4, 5, Fig. I and Table 1 of Ref~. 1.3.
ACTIVITY 4 -Wiring is mounted and supported such that minimum separation will be maintained (Ref. 1.1, para. 2 and Ref. 1.3).
ACTIVITY 5 -The correct type barrier material is installed (Ref. 1.1, Note).
ACTIVITY 6 -Verify that " required" barriers are installed.
Note: " Required" means either required by a design document, or indications that a barrier was
, installed but has been removed.
The inspector will state the location of missing barriers and whether or not a separation violation has been created by the barrier's absence.
4.2 For Sections C and D only of the inspected control boards, the Quality Inspector shall identify all exposed non-Tefsel wiring (Ref. 1.1, para.
I and Ref. 1.2).
4.3 DOCUMENTATION 4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents required to perform activities in paragraphs 4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results and inspections for para. 4.1 on the IR (Attachments 2 and 3, as applicable). For para. 4.2, the Quality Inspectors shall identify exposed non-Tefzel wiring on Attachment 4 (Supplement Sheet) and attach the form to the appropriate IR.
QI-004 R visient 4 Page 3 of 3 4.3 DOCUMENTATION (Cont'd) 4.3.3 The inspection documents (Attachments 2, 3 and 4) shall be completed in accordance with their respectiva ccaplaticn instructions, as applicable. The inspector completing the report shall indicate that all applicable inspections have been completed and documented correctly by entering signature, date and badge number. The IR's shall be reviewed for legibility, accuracy, and completeness and approved by the ERC Level III. The completed IR shall be submitted to the Electrical / Instrumentation Review Team Leader for review and approval.
4.3.4 Per Reference 1.1, Para. 5, the inspection results of panels XCV-01 and XCV-03 will indicate that Section A/B criteria and inspections 4 are not applicable. All sections of panels ICB-10 and 11 will be inspected to C/D criteria and documented accordingly.

14 . 0 INSTRUCTION 4.1 'Ine, Quality Inspector shall inspect the specific Main Control boards and vertical panils-(Ref. 1.1) to verify-that:

l 4.3 DOCUMENTATION l-4.3.1 The Quality Inspectors will be furnished with the applicable L- drawings and documents required to perform activities in paragraphs b 4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results and inspections for para. 4.1 on the IR (Attachments 2 and 3, as applicable). For para. 4.2, the Quality Inspectors shall identify exposed non-Tefzel wiring on Attachment 4 (Supplement Sheet) and attach the form to the appropriate IR.

The IR's shall be reviewed for legibility, accuracy, and completeness and approved by the ERC Level III. The completed IR shall be submitted to the Electrical / Instrumentation Review Team Leader for review and approval.
4.3.4 All section a and b criteria blocks and inspection results blocks-for panels ICB-10, ICB-11, XCV-01 and XCV-03 will be NA'd per,Ref.
1.1, para. 5. All section c and d criteria and inspection results blocks shall be completed as appropriate.

Jones (CPP-17,914) 2.0 PURPOSE AND SCOPE 2.1 The purpose of this Quality Instruction is to define the inspection activities, methods, and documentation requirements necessary to assure that adequate physical separation of wiring has been maintained, and that required barriers have not been recoved without authorization. This effort is in direct support of CPRT Action Items I.b.1 and I.b.2.
2.2 The scope of this procedure is the cabling and wiring within selected control 2 panels and cabinets as identified in the above references. The scope of inspections is any interaction between safety related wiring and components and other safety related or non-safety related wiring and components, excluding vendor wiring to vendor wireway (per Ref.1.5).
NOTE: The scope of this Quality Instruction may be expanded at the direction of the Electrical / Instrumentation Review Team Leader.
3.0 RESPONSIBILITIES 3.1 The third party inspectors (ERC) shall be responsible for conducting inspections and for completion of inspection reports. The Electrical / Instrumentation Review Team Leader is responsible for the review and approval of the inspection reports.

' -.4.1 The Quality Inspector shall inspect tne specific Main Control boards and vertical panels (Ref.~1.1) to verify that:
ACTIVITY 1-- Internal lighting and 120 VAC convenience receptacle wiring not-routed in rigid conduit, EMT, or Unistrut P-1000 channel -is separated at least 6 inches from safety related wiring and components (Ref.1.1, para. 3 and Ref.1.5).
- ACTIVITY 2 - Flexible conduit attached to control switch modules does not exceed
, 3 feet in length without additional support (Ref.1.1, para. 2 and Ref. 1.3).
ACTIVITY 3 - Separation is maintained in accordance with Ref.1.1, para. 4 and 5 and Figure 1, Ref.1.3, Table 1 (Attachment 1).
ACTIVITY 4 - Wiring is mounted and supported such that minimum separation will-be i maintained (Ref.1.1, para. 2 and Ref.1.3).
ACTIVITY 5 - The correct type barrier material is installed (Ref.1.1, Note).
ACTIVITY 6 - If it is obvious that separation barriers have been removed (i.e.
I brackets installed but no barriers, etc.), or if design drawings j' require barriers that are not installed, verify that these barriers
[ have not been removed without authorization (Ref.1.4). .

4.3 DOCUMENTATION 1
4.3.1 The Quality Inspectors will be furnished with the applicable drawings and documents
! required to perform activities in paragraphs 4.1 and 4.2.
4.3.2 The Quality Inspectors shall document the results of inspections for para. 4.1 on ,
the IR (Attachments 2 and 3, as applicable). For para. 4.2, the Quality Inspectors o shall identify exposed non-Tefzel wiring on Attachment 4 (Supplement Sheet) and
- attach the form to the appropriate IR.
) 4.3.3 The inspection documents (Attachments 2, 3 and.4) shall be completed in accordance j with their respective completion instructions, as applicable. The inspector com-plating the report shall indicate that all applicable inspections have been completed ,

2.1 The purpose of this Quality Instruction is to define the method by which *-
to ensure that required separation is maintained within specified Main Control room panels and that no separation barriers have been removed with-out authorization. This is in direct support of the CPRT Action Items I.b.1 and I.b.2.
2.2 The scope of this Quality Instruction is to verify by impection, that required separation is maintained and that no separation barriers have been removed without authorization.
3.0 RESPONSIBILITIES 3.1 The third party inspectors (ERC) shall be responsible for conducting inspec-tions and for completion of inspection reports. The Electrical /Instru-mentation Review Team Leader is responsible for the review and approval of the inspection reports.
4.0 INSTRUCTION I 4.1 The Quality Inspector shall inspect the specific Main Control boards and l vertical panels (Ref.1.1) to verify that:
, Activity #1 - Wiring is mounted and supported such that minimum separation l will be maintained (Ref.1.1, para. 2).
l Activity #2 - Flexible conduit attached to control switch modules does not exceed 3 feet in length without additional support (Ref.1.1, para .2 ).

Activity #3 - Internal lighting and 120 VAC convenience receptacle wiring i is routed in rigid conduit, EMT, or Unistrut P-1000 channel (Ref.1.1, para 3).

Activity #7 - The correct type barrier material is installed. (Ref.1.1,

Activity #8 - If it is obvious that separation barrie'rs have been removed (i.e. brackets installed but no barriers, etc.), or if design drawings require barriers that are not installed, verify that these barriers have not been removed without authorization.
Activity #9 - If barriers have been removed or are not installed, verify that documents exist requiring installation / reinstallation of the barrier (s).
4.2 For Sections C and D only of the inspected control boards, the Quality Inspector shall identify all non-Tefzel wiring (Ref.1.1, para 1).
4.3 DOCUMENTATION i
