MODIFICATION NO. 2 TO TASK ORDER NO. 6 ENTITLED " MODIFICATIONS TO THE HUMAN FACTORS INFORMATION SYSTEM (HFIS)" UNDER CONTRACT NO. NRC-03-97-022
Dear Mr. Nock:
The purpose of this modification is to: (1) extend the period of performance of the subject task order through April 30,1999, at no additional cost or obligation to the Government and (2) revise the completion date for Activities 1 through 5 from " January 31,1999" to " April 30,1999." The extension is necessitated due to the NRC computer network staffinstalling NT, thereby enabling the contractor to perform the Activities 1 through 5, which are identified in the Statement of Work to the basic task order. The extension will allow the contractor adequate time for the successful completion of all work. The effective date of this modification is March 5,1999. Therefore, any work performed during the period of February 1,1999 through March 4,1999 will not be reimbursed.
In addition, on page 2 of the Statement of Work of the basic task order, the completion schedule for Activities 2 through 5 is revised from " January 31,1999" to " April 30,1999."
In accordance with Section G.4(c) of the subject contract entitled " Task Order Award," this letter definitizes Modification No. 2 to Task Order No. 6.
The second paragraph, first sentence of the definitization letter of the basic task order, the
'O expiration date of Task Order No. 6 is revised to read " April 30,1999."
The issuance of this modification does not amend any other terms and conditions of the subject task order, including the ceiling amount of $3,002.25.
A summary of obligations for the subject task order, from award date through the date of this action, is given below:

$3,002.25 This modification does not obligate any FY99 funds.
