Dear Ms. Beckman:
The purpose of this modification is to revise the schedule for the tasks under this task order, as shown in " bold" on the enclosed revised Statement of Work; increase the level of effort; and change one of the " Key Personnel" under the task order. Therefore, Task Order No. 020 is modified as follows:
Task Order No. 020 shall be in effect from November 9,1998 through April 301999. The cost ceiling of this task order is increased by $16,873.96 from $76,773.35 to $93,647.31. The amount of $90,699.57 represents the estimated reimbursable costs, and the amount of $2,947.74 represents the fixed fee.

BACKGROUND Tha licensee (Northeast Utilities) has taken numerous steps during this extended outage to address Millstone Unit 2 l
performance problems. The purpose of this task is to obtain assistance in the completion of the NRC actions necessary '
to conclude that adequate performance improvement has resulted from various licensee initiatives to support a safe plant I r: start end continued operation.
A revi:w and inspection will be conducted for Millstone Unit 2 at the plant site in Niantic, CT. The inspection will assess the operational readiness of the Millstone Unit 2 plant. The inspection activities will be performed in accordance with NRC l Inspection Procedure 93802.
OBJECTIVE l
i Th3 objective of this task order is to obtain the expertise of two specialists who have extensive experience evaluating the l eff:ctiveness of nuclear power plant programs and oversight to ensure operational safety. Specifically, the contactors {
will cssist in assessing the areas of operations, self-assessment, independent oversight, quality review grouos, l man:giment processes, corrective actions and start-up plans. l 1
Th3 specialists are needed to assist the NRC staff in the review and evaluation of the Millstone Unit 2 restart readiness. i Tha focus of the inspection is to perform an integrated evaluation of the overall condition of the site and to assess the )
readin:ss of the licensee to operate the facility. The specialists should be thoroughly familiar with NRC regulations and ;
inspection methodology. !
It shall be the responsibility of the contractor to assign technical staff, employees, and subcontractors, who have the requir:d educational background and experience to meet both the technical and regulatory objectives of the work specified in this SOW. The NRC will rely on representation made b'ylife contractor conceming the qualifications of the personnel proposed for assignment to this task order including assurance that all information contained in the technical and cost proposals, including resumes and conflict of interest disclosures, is accurate and truthful.
WORK REQUIREMENTS AND SCHEDULE l Ths contractor shall provide the qualified specialists, and the necessary facilities, materials, and services to assist the NRC staff in preparing for, conducthg, and documenting the inspection activities and findings. The Technical Monitor for this task is Jim Trapp. The Technical Monitor may issue technical instructions from time to time during the duraticn .

- 3. Prepare inspection report input. Documentation of the inspection activities, findings and results is to take place at the Region I office on or about March 29 - April 2,1999. Final report input is due on or about April 6,1999.
NOTE: Prior to the start of on-site inspection, the contractor's staff is required to be avalL'ble to coordinate Nection aspects, such as travellogistics, with the Team Leader.
REPORTING REQUIREMENTS i Technical Reoorts At the completion of Task 1, the contractor's specialists shall provide inspection plan input to the NRC Team Leader. The format and scope of this input shall be as directed by the NRC Team Leader.
During Task 2, each contractor's specialist shall provide daily reports to the NRC Team Leader. The format and scope of these reports shall be as directed by the NRC Team Leader.
. At the completion of Task 2, (prior to the inspection team's exit meeting) each contractor's specialist shall provide a draft inspection report input to the NRC Team Leader. Again, the format and scope of this input shall be as directed by the NRC Team Leader. Typically, this input will consist of a summary of each specialist's inspection findings.
At the completion of Task 3, the contractor shall deliver the final inspection report input (feeder report) to the NRC Project Officer (original and one copy) with one hard copy and one computer diskette version (Wordperfect 5.1/6.1 or other IBM PC compatible software acceptable to the NRC Team Leader) to the NRC Team Leader.
The format and scope of the final report inputs shall be in accordance with the guidance in NRC Inspection Manual Chapter 0610 or as directed by the NRC Team Leader.
A specialist's feeder report will serve as documentation of the specialist's inspection activities, effort, and findings, and will be used by the NRC Team Leader for the preparation of the NRC's inspection report. The form and scope of the final report input shall be in accordance with the guidance in NRC Inspection Manual Chapter 0610 or as directed by the NRC Team Leader. As a minimum, each specialist's report input shallinclude the following:

o For each area inspected, a description of the activities and general findings and conclusions reached regarding ;

o For each area with a concern or findings, a discussion of the concerns or findings with technical bases.
NOTE: The contractor is not required to undertake any further efforts toward report finalization. For example, management review of the feeder report beyond its submittal to the NRC Team Leader and Project Manager is
, not needed.
Business Letter Reoorts The contractor shall provide monthly progress reports in accordance with the requirements of the basic contract.
MEETINGS AND TRAVEL For estimating purposes, the following meetings and travel are anticipated:

OTHER APPLICABLE INFORMATION I The work specified in this SOW is 100% licensee fee recoverable. The contractor shall provide fee recovery information in the monthly progress reports in accordance with the requirements of the basic contract. plant site. The contractor shall provide all documentation required for badging (as identified by the NRC Team Leader) at the plant site. On-site Inspection. of this task order. Questions concerning badging and the plant site access shall be addressed to the NRC Technical Monitor.
