s NOTICE OF VIOLATION Illinois Power Company License No. CPPR-137 As a result of the inspection conducted on February 24 through April 7,1986, and in accordance with the General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1085), the following violation was identifed:
10 CFR 50, Appendix B, Criterion V, as implemented by Illinois Power Company Operational Quality Assurance Manual (0QAM) requires that activities affecting quality shall be accomplished in accordance with procedures.
Procedure CPS No. 1005.07, Revision 4, Temporary Changes to Station Procedures, requires:
(a) in paragraph 2.2.2.2, that any temporary changes to a procedure shall be limited to those contained in an approved Procedure Deviation for Revision (PDR);
(b) in paragraph 8.1.2, that any approved temporary change shall be necessary to continue work in progress and the work in progress shall be essential to maintain plant availability or to ensure plant nuclear safety; and (c) in paragraph 8.1.4, that a temporary change shall not be used to alter the intent of the original procedure as defined in the procedure purpose section.
Contrary to the above, on March 12, 1986, activities affecting quality were not accomplished in accordance with procedure CPS No. 1005.07 in that the temporary change to CPS No. 1029.01, Revision 7, Preparation and Routing of Maintenance Work Requests (a) was more extensive than described in PDR 86-094; (b) was not necessary to maintain plant availability or to ensure plant nuclear safety; and (c) altered the intent of the original procedure as defined in the procedure purpose section by extending the coverage of the procedure to other than processing work requests and implementing corrective actions.
This is a Severity Level IV violation (Supplement II).
