Dear Mr. Keppler:

On April 18, 1986, Mr. Rod Cook of Toledo Edison notified Mr. Nick Jackiv of NRC Region III of a potential 10CFR21 concerning ITT General Controls Hydramotor Actuators (Models NH91/95). These devices are utilized as Ventilation System Damper Controls that modulate ambient atmosphere conditions in areas where safety-related equipment such as the Emergency Diesel Generator and Component Cooling Water Pumps and associated instrumen-tation are located.

There are 22 ITT General Controls Hydramotor Actuators in use in safety-related applications at Davis-Besse. Table I identifies the location of the subject components installed at this facility.

Based upon the investigations conducted to date, Toledo Edison has experienced leakage of hydraulic fluid from two of thirteen of these actuators that have been recently tested. This leakage could result in making the hydramotor actuator inoperable. The causes of the observed leakare are currently being evaluated by the Toledo Edison Company to determine the extent of the deficiencies and corrective actions. This evaluation is expected to be completed by June 2, 1986.

This letter is being submitted to confirm the April 18 telephone discussion and to inform the NRC that we are continuing to evaluate the situation. Upon completion of our evaluation, we will inform you of the results and, if required, provide appropriate notification as specified in Title 10.
