The Ra.ii.cion Safety Officer who answers directly to the Q. A.

Manager is responsible for all documentation, training, certification and auditing of radiographic personnel and Q. A. program pertaining to 10 CFR part 20, 34, 71.

4.

The Radiation Safety Officer, Assistant R. S. O. and radiographers are responsible for handling, storing, shipping / receiving and inspection testing documentation of all radioactive sources.

5.

Control of design and fabrication of radioactive material shipping and handling devices and packages shall not be conducted at South-western Engineering Co.

Quality Assurance Program 1.

Southwestern Engineering Co. (SEC0) establishes all criteria contained in Quality Assurance program.

2.

Q. A. program revisions and procedures will be made only upon management approval.

The Q. A. program will have all control of characteristics of f

package dealing with safety.

All training for functions dealing with Q. A. will be documented to written procedures.

5.

The Q. A. program will ensure that all approved Q. A. procedures, engineering procedures and special provisions of package design (O

The Radiation Safety Officer (R.S.O.) shall ensure that all radio-active material shipping packages are designed and manufactured under a Q. A. program approved by the Nuclear Regulatory Commision (N. R. C. ).

Packages will be documented by means of certification from manufacturer.

Document Control 1.

The Radiation Safety Officer shall insure that all Q. A. functions are conducted in accordance with most current revisions applicable.

2.

All document changes or revisions will be performed according to written procedures approved by SEC0 management.

All documents relating to a specific shipping package will be controlled through the use of written procedure.

Instructions, procedures and drawings.

1.

All special instruction, standard operating procedures and drawings relating to specific or identified packages will be indoctrinated into SEC0 Q. A. program only after management approval.

2.

All instructions, procedures and drawings will contain all pertinent information needed to perform specific functions.

Handling, Storage and Shipping 1.

Documented safety procedures concerning the handling, storage and shipping of packages for certain special form radioactive material will be followed as Standard Operating Procedures.

2.

Prior to shipment of any radioactive material, an inspection to verify that all tests, certifications, acceptances and final inspection have been completed and have met acceptable criteria.

Operating instructions will be provided for handling, storing and shipment of all packages.

4.

Only radiographic personnel shall perform the critical handling, storing and shipping operation.

G.

Inspection, Test and Operating Status 1.

Inspection, testing and operating status of packages for certain special form radioactive material will be indicated and controlled according to written procedures.

2.

Status will be indicated by tag, label, marking or log entry.

3.

Status of nonconforming parts or packages will be postively maintained by written procedures.

4.

The Assistant R. S. O. and all radiographers shall be certified to perform all regulatory required inspections and test in accordance with written procedures.

5.

The R. S. O. shall ensure that these required functions are performed and documented.

H.

Quality Assurance Records 1.

All documentation pertaining to inspections, tests, operating logs, personnel training, audits and qualifications and record of shipment will be maintained by R. S. O.

Descriptions of equipment and written procedures will also be maintained.

3.

All records shall be maintained in accordance with written procedures and all records shall be traceable and retrievable.

4.

A list of these records, with their storage locations shall be maintained by the Radiation Safety Officer.

Audits 1.

Established schedules of audits of the Quality Assurance Program will be performed using written checklists.

2.

The audits will be dependent on the safety significance of the activity being audited, but each activity will be audited at

Results of audits will be maintained and reported to management.

Audit reports will be evaluated and deficient areas corrected.

5.

Audit reports will be maintained as part of the quality assurance records.

6.

Members of the audit team shall have no responsibility in the activity being audited.
