Dear Mr. Montgomery:

This is in response to Notice of Violation, dated August 6,1986, pursuant to the provisions of 10 CFR 2.201. The notice identified one item which is repeatec below followed by our response.

Violation The licensee's approved " Physical Protection Plan", dated i:ovember 30,1982 at Section 1.1.l.3, reads in part: "The controlled access arco perimeter fence is lighted by 400 watt mercury vapor lights mounted at a height of 32 feet on wooden poles spaced approximately 140 feet apart."

Contrary to the above, it was observed on the evening of July 28, 1986, that lights on three poles at the perimeter adjacent to the Security Guard Station at the west Controlled Access Area (CA A) barrier, directly south of Office Building #2, were inoperative.

Response

The cause of the inoperative lights was a failure of the feeder cable. An extended period before the lights were repaired was caused by the following:

a.

Both the Exxon Nuclear Electrical Foreman and City of Richland personnel failed to recognize that the inoperative lights ~were perimeter lights.

The repair period was being based on the assumption of failed parking lot lights only and a desire to bypass the f ailed point of the underground feeder rather than use temporary overhead cable.

Once it was evident that perimeter lights were involved, repairs were made promptly and completed on July 29, 1986.

b.

Failure of security personnel to keep reporting that the failure had not been repaired and of security management to follow-up properly.

a.

Security personnel have been instructed that inoperative lights will be listed in the security activites log when discovered and will continue to be listed each day until repaired.

b.

The Manager, Plant Security has been instructed to track the reporting and repairing of any inoperative lights.

c.

All personnel. have been instructed to distinguish between perimeter and parking lot lights in order that perimeter light repairs con be expedited.
