We have reviewed your application dated September 4,1985 requesting a new
' license and find that we will need additional information as follows:
1.
Please submit a description of how access to the gauge will be controlled (e.g., warning signs, barriers, inaccessible areas, etc.).
2.
Please submit a description of the precautions, restrictions, and " lock-out" procedures (i.e., procedures for preventing employees from entering the radiation beam during maintenance, repairs, or other work on or around the area where your gauge is mounted) that you will use.
If the gauge shutter or switch is locked, bolted, etc., until the work is completed, you should describe this and provide the name of the individual (s) responsible for enforcing this procedure.
3.
If you wish to provide in-house training for your own personnel, a description of the training must be provided.
Included in the description of in-house training should be:
The name(s), training and experience of the individual (s) providing a.
fonnal training.
b.
An outline of the formal training and on-the-job training to be provided, including the duration of the training.
c.
The means of determining when the trainee has satisfactorily completed the training and is capable of carrying out the radiation safety responsibilities required by the licensee.
Since it appears that you have a limited amount of gauges, your program does not warrant the generic authorization to allow any Kay-Ray gauge.
Please submit the model number of the sealed source and the model number of the source holder you will be using and how many gauges you will have on-site.
