This refers to the inspection conducted by K. R. Naidu, J. B. Jacobson, and E. W. Merschoff of this office on November 11-14, 1985 and January 13-16, 1986, of your facilities at Seco Road and Murray Center, Monroeville, Pennsylvania and to the discussions of our findings with Mr. J. Epstein and other members of your staff at the conclusion of the inspection.

The purpose of this inspection was to review actions taken by you to evaluate the failures of UnderVoltage Trip Attachments (UVTAs) on Reactor Trip Breakers at the D.C. Cook Unit 2 and Kewaunee nuclear power plants, including the observation of tests conducted at the Seco Road facility during November 11-14, 1985. Corrective action taken to resolve one construction deficiency related to broken spring release latches observed in 480 volt circuit breakers was also reviewed during this inspection. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspectors.

We noted that you have completed the tests on the UVTAs which failed at the D.C. Cook plant, and you fomarded the results to the D.C. Cook station. Based on these preliminary results, we note that you have issued a Maintenance Advisory Letter on November 21, 1985, describing the O.C. Cook event and advising users to visually inspect UVTAs for surface anomalies such as weld beads or notches in the leading edge of the UVTA base.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
