Due to the degradation of the associated fire barriers, this report is being submitted for informational purposes in accordance with the requirements of Technical Specification Action Statement 3.7.11.a.

, Outage - Planned Evolution Event Date: 03/28/86 Report Date: 04/25/86 This report was initiated by Incident Report No.86-093 CCBIDITIONS PRIOR TO OCCURRENCE:

Refueling Outage DESCRIPTION OF OCCURRENCE:

This special report describes the circumstances surrounding the degradation of fire barriers due to certain fire doors being impaired for the duration of the refueling outage. This report is being submitted for informational purposes in accordance with the requirements of. Technical Specification Action Statement 3.7.ll.a, which states:

With one (1) or more of the required fire barrier penetrations

, non-f unctional, within one (1) hour either establish a continuous

fire watch on at least one- (1) side of the affected penetration, j or verify the operability of fire detectors on at least one (1) side of the non-functional fire barrier and establish an hourly
fire watch patrol. Restore the non-functional fire barrier penetration (s) to f unctional status within seven (7) days or, in lieu of any other report required by Specification 6.9.1, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next thirty (30) days outlining the action taken, the cause of the non-functional penetration (s) and plans and schedule for restoring the fire barrier penetration (s) to functional status.

i On March 28, 1986, impairments were issued to allow certain fire doors to be blocked open for the duration of the Unit 1 sixth refueling outage. These particular doors are located in high traffic areas of the plant, and the impairments vere issued to preclude damage which could be sustained due to excessive use experienced during the outage.

All of the effected doors are within Unit 1 proper, and none of the doors connect to the operating unit (Unit 2). Continuous fire patrols wcre established by Site Protection on March 14, 1986, due to increased work activities resulting f rom outage preparations; i.e. ,

the outage commenced on March 22, 1986.

These fire patrols will be maintained until such time as the doors are returned to an operational status. The refueling outage is presently scheduled for completion on May 3,1986.

, The functional integrity of the penetration fire barriers ensures that i fires will be confined or adequately retarded f rom spreading to adjacent portions of the facility. This design feature minimizes the possibility of a single . fire rapidly involving several areas of the facility prior to detection and extinguishment. The penetration fire barriers are a passive element in the facility fire protection program

and are subject to periodic inspections. The inoperable fire doors i constitute a breach of the associated fire barriers; however, ,

l appropriate actions were taken to comply .with the requirements of Technical Specification 3.7.11. This action provides adequate i assurance that a fire will be readily detected and extinguished; thus,

~

alleviating the possibility of a fire affecting adjacent portions of the facility. This was a planned evolution. The fire doors are mechanically functional and could be cleared of any obstuctions and I closed if required. This occurrence therefore involves no undue risk 3 to the health or saf ety of the public. However, because the fire

, doors were not returned to an operable status within the seven (7) days specified by Technical Specification 3.7.11, this special report is being submitted pursuant to the requirements of Technical Specification 6.9.2.

! Technical Specifications 3.7.10.1 (fire suppression water systems),

3.7.10.2 (spray and/or sprinkler systems) , 3.7.10.3 (low pressure CO2 systems), 3.7.10.4 (fire hose stations), 3.7.11 (cable penetration i

barriers, fire doors and fire dampers) and 3.3.3.6 (fire detection

instrumentation) require special reports to be submitted if equipment  ;

I is inoperable for a specified period of time. These specjfications provide adequate alternative measures to ensure that fires will be

, readily detected and extinguished. Providing that the action

! requirements are complied with, these Specifications do not require i Unit power reduction or shutdown (regardless of how long the equipment is out of service). In the future, reports will not be submitted for

" planned evolutions" involving the inoperability of fire detection,
suppression or protection equipment for Unit 1 or Unit 2, as long as the action requirements are met. These occurrences will, however, be 4

documented and controlled in accordance with existing Administrative i

Procedures. Reports will be submitted for inoperable equipment due to reasons other than " planned evolutions"; i.e., a damaged fire door not repaired in the seven (7) days required by Specification' 3.7.11, a non-functional fire suppression water pump discovered during routine surveillance which is not restored to an operable status within the j seven (7) days required by Specification 3.7.10.1, a fire hose station

! inoperable for greater than fourteen (14) days due to an inoperable valve, etc.
