NOTICE OF VIOLATION Illinois Power Company License No. CPPR-137 As a result of the inspection conducted on July 29 through August 11, 19c6, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the following violation was identified.

10 CFR 50, Appendix B, Criterion XVI, as implemented by the Illinois Power Company (IP) Nuclear Power Operational Quality Assurance Manual (00AM),

requires in part that each IP organization have a documented corrective action system which assures that conditions adverse to quality are promptly ,

identified, analyzed for causes, corrected to preclude recurrence, and followed up to verify proper implementation of corrective action, and that the corrective action is closed out in a timely manner.

Contrary to the above, corrective actions taken by the IP plant staff to address the generic implications of IP QA Audit Q38-86-10, finding ID-1, concerning the training of plant staff personnel to procedure and program changes 1) were not pro ptly taken to preclude recurrence, and 2) were not implemented prior to closure of the audit finding, as detailed in Inspection Report 50-461/86055, paragraph 2.e.

This is a Severity Level IV violation (Supplement II).

Pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each violation: (1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
