NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-254

( Docket No. 50-265 As a result of the inspection conducted on June 8,1986 through August 9, 1986, and in accordance with the " General Policy and Procedures for NRC Enforcement Actions,' 10 CFR Part 2, Appendix C (1985), the following s

violation was identified:

Technical Specifications Section 6.2 requires that detailed written procedures, including applicable checkoff lists shall be prepared, approved and adhered to for preventive and corrective maintenance which could have an effect on the safety of the facility; normal startup, oeeration, and shutdown of the reactor and systems and components involving nuclear safety of the facility; and Surveillance and Testing requirements.

Contrary to the above, during the July 9,1986 startup, the following events occurred on Unit 1:

1. The Unit 1 operator failed to verify that all rods in the group he had

' moved were ct their target out position prior to moving to the next group (QGP 4-1 Revision 3: Control Rod Movements and Control Rod Sequences StepD.5.a.5). This resulted in one rod not being at the correct position.

2. The Qualified Nuclear Engineer failed to return an out-of-sequence control rod to its in-sequence position by the method approved in station procedures (QTP 1600-5 Revision 3: Returning Out-Of-Sequence Control Rods to their In-Sequence Positions - Step F.2.G). However, this resulted in no adverse effects to the reactor core.

! This is a Severity Level IV violation.

The inspection showed that action had been taken to correct the identified violation (s) and to prevent recurrence. Consequently, no reply to the I violation (s) is required i.nd we have no further questions regarding this matter. ,
