Task Order No. 5, Modification No. 2 under Contract No. NRC-04-96-036 Entitled
" Technical Assistance Related to ISA issue No. 5" The purpose of this modification is to: (1) provide for a within scope char.ge to which revises subtasks 5.1 and 5.2 and eliminates subtask 5.3. , (2) increase the task ord6r ceiling amount by $18,407 from $84,545 to $102,952 and the total amount obligated by $18,015 from $84,545 to $102,560 and (3) extend the period of performance. This effort shall be performed in accordance with the contractor's technical proposal dated December 30,1097. Accordingly, the following changes are hereby made:
1.) Task 5 of this task order is revised in accordance with the attached statement of work and the contractor's proposal dated December 30,1997, 2.) The total estimated cost for Task Order No. 5 is $102,952. The amount of $95.705 represents the total reimbursable costs and the amount of $7,247 represents the fixed fee.
The amount currently obligeted by the Govemment with respect to this task is $102,560, of with the sum of $95,405 represents the estimated reimbursable costs, and of which $7,155 represents the fixed fee. It is estimated that the amount currently allotted will cover performance through July 30,1998.
3.) Task Order No. 5 sha,1 be in effect from August 11,1997, through July 31,1998.
