Your Georgia License No. 1153-1, is recognized as valid for use in Non-Agreement States and areas of Exclusive Federal Legislative Jurisdiction for ANY 180 DAYS IN CALENDAR YEAR 1998 ONLY. We have assigned a Location Reference Number (LRN. NRC Form 241. Item 16) to each temporary job site you have reported. Activities which have not been previously assigned an LRN are
-considered revisions. should be reported on the enclosed NRC Form 241 and marked as a Revision (Item 2) Revisions should be faxed to NRC_ Region II three days _ in advance of the-proposed activity, along with a coay of the fee
- for a Revision. There is no limit to the number of activities reported at one time, and one: fee is charged without regard to the' number of activities on Revision. Revisicns will be reviewed and acknowledged includir.g assignment of an LAN for the job location.
