Dear Mr. Kinneman:
This letter constitutes AltCO's response to the Notice of Violation issumi by tho N ieur llegulatory Conunission on.lanuary 12,1998. The Notico of Violation resulted fnnn the inspection of the Itoy I?, Weston facility usml to store amt receive AltCO Nuclear Medical Prodtickr Osmpany pacemakers after they are explanted from patients. The Notice of Violation contained t,wo v!olations. In tho Orst instance, AltCO sulunittmlits Omdition 12 Ileport at an interval greater than 12 months fnnu tho submittal of the previous report.
The wcond violation occurrm! when AltCO's contractor, Itoy I?. Weston, Inc. (Weston) failed to emuluct sampling of the storngo aren during tho thint quarter of 1997. AllCO's license requires sampling once during each quarter.
in onler to comply fully with the terms ofits lic9nse, AltCO has revised the management of the pacemaker progr,im. AltCO has developed a project whmiule for uso during 1998 and subseqtient years. The project schedulo includes: Submittal of the Comlition.12 Iteport, comluct of the 8emi annual patient survey, comluct of the semi annual DOE inventory aml the quarterly monitoring of the sinrage aren.
