This refers to the inspection conducted by Mr.- J. J. Petrosino of this office on December 9-13, 1985, of your facility at East Bridgewater, Massachusetts and to the discussions of our findings with Mr. M. J. Berberian and members of your staff at the conclusion of the inspection.
The purpose of this inspection was to review the circumstances surrounding the conductor insulation degradation of a 15 conductor retractable cable. This cable was a part of the Foxboro supplied "E line" controllers. Areas examined during the inspection and our findings are discussed in the enclosed report.
Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
During the inspection it was found that the implementation of 'your QA program failed to meet certain NRC requirements.
Inadequate implementation of your QA procedures was apparent in the areas of indoctrination, 10 CFR Part 21 procedures, internal in-process audits, and a lack of 10 CFR Part'21'reportability evalua-i tions for several NRC licensee.related problems which were recorded on Foxboro failure analysis reports. The most significant of these was the failure of The Foxboro Company to perform the' required 10 CFR Part 21. notifications. This was evidenced by a'1979 Foxboro report (ref. #CFA-28), in which the conductor insulation disintegrated on all-15 conductors of an "E line," interconnection cable set. This insulation disintegration problem was identified to Foxboro in the fall of 1978. The specific findings and' references to the pertinent requirements are identified in the enclosures to this letter.
The enclosed Notice of Violation is sent to you pursuant to the provisions of.
Section 206 ~ of the Energy Reogranization Act of 1974.. 'You are required to submit to this office within 30 days from the date.of this letter a written statement containing:
(1) a description of steps that have been or will be taken to correct these items;-(2) a description of steps that have been or will be taken to prevent recurrence;.a'nd-(3) the dates your corrective actions and preventive ceasures were or'will be completed. Consideration may be given to extending your response time for good'cause shown.
