The Axsym, from Abbott Laboratories, was installed in the on-site testing facility to improve the overall effectiveness of the badging process.

Lessons Learned On December 1,1998 a blind proficiency specimen spiked with Benzodiazepines was submitted to the DilllS Certified Laboratory for testing. The DlHIS Laboratory reported the specimen as

- negative. This specimen had previously screened positive for Benzodiazepines in the on-site testing facility. An investigation was initiated to determine the cause for the difference in the results. This concern was reported to the NRC in accordance with 10 CFR 26, Appendix A, 2.8(e).- The investigation determined that the cause of the false negative result was a clerical error

. at the Dillis Laboratory.

Reportable Fitness for Duty Events Under 10 CFR 26.73 There were no reportable Fitness for Duty events during this reporting period.
