Contract No. NRC-26 94-263, Task Order No.11 In accordance with Section G.3 entitled, " Task Order Procedures" of the subject contract, this letter definitizes Task Order No. 11. The effor, shall be performed in accordance with the enclosed Statement of Work.

Task Order No.11 shall be in effect from March 9,1998 through September 30,1998, with a ceiling of $95,501.00. The amount of $85,764.00 represents the total firm-fixed-price for the course as stated in the enclosed Statement of Work. The amount of $9,737.00 represents the total estimated reimbursable costs for travel associated with this task order.

a TASK ORDER NO.11 UNDER CONTRACT NO. NRC-26-94 263 Root Cause / Incident Investigation Workshop - FY 1998 1.

PURPOSE The Nuclear Regulatory Commission (NRC) licenses and inspects reactor and non reactor facilities to ensure compliance with applicable codes and standards during all phases of construction, testing, and operation in order to identify conditions which may adversely aff.ct the health and safety of the public, in order to ensure that the investigation of events is carried o it in a structu.ed, coordinated and integrated manner, the NRC's Office for Analysis and Evaluation of Operational Data provides formal training in incident investigation.

The primary objective of Task Order 11 under contract NRC 26 94 263 is to provide training necessary to ensure that operational events are investigated in a systematic and technically sound manner, to gather infortnation pertaining to the root causes of the events, including any NRC contributbns or lapses, and to provide appropriate feedback regarding the lessons rf experience to the NRC, industry, and to the public.

The purpose of this task order is to establish course dates for the Root Causellncident investigation and Human Performance Investigation Process Workshops.

2.

SCOPE OF WORK 1)

Establish the following course dates for Root Causelincident Investigation Workshop:

Region 2 March 16 - 20,1998 Region 3 April 20-24,1998 Headquarters June 8 - 12,1998 Region 1 June 1-5,1998 2)

Establish the following course date for Human Performance investigation Process Workshop:

Region 3 May 7,1998 3.-

ME2 TINGS No meetings are anticipated.

Travel Travel required in accordance with section G.2 and section C.3.3.9.b.1 of the

. contract to support course presentations listed above.

5.

REPORTING REQUIREMENTS / DELIVERABLES a.

C.:iverables. The contractor shall:

1)

Present the Root Cause/ Incident Investigation Workshops on the prescribed date and location.

2)

Present the Human Perfo_rmance investigation Process Workshops on the prescribed date and location.

b, Milestones 1)

Present courses on the prescribed date.

Within 30 days of completion of a course the contractor shall submit a course presentation report.

c.

Reporting requirements The contractor shall submit a course presentation report in accordance with Section F of the contract.

6.

PERIOD OF PERFORMANCE The period of performance of this task order shall be frcm the date of issue through, Septem'oer 30.1998.

7.

TECHNICAL SKILLS REQUIRED l

The contractor shall provide resumes of all personnel selected to work on the project unless provided previously. Specific skills required include familiarity with the existing Root Causellncident Investigatior S/; ~ 1 hop, Root Cause/ incident Investigation Refresher Workshop, and Humar caormance Investigation Process Workshop.

GOVERNMENT PROPERTY TO PE PROVIDED TO THE CONTRACTOR None.
