Enclosed is a copy of revised NRC Form 398, " Personal Qualification Statement - Licensee,"
(10/97) Enclosure 1, which has been revised to reflect current operator licensing policy.
By copy of this letter, your facility training manager is being provided with a small supply of the form and is asked to ensure that license applications are submitted on the revised forms ss soon as possible but no later than May 1,1998.
