Inspection Conducted March 31 through April 4, 1986 (Report 50-267/86-09)

Areas Inspected: The Region IV team made a routine, announced inspection of

licensee maintenance activities, covering the maintenance program, maintenance

program impleneentation, instrumentation and control (I&C) maintenance, and

electrical maintenance.

Results: Within the four areas inLpected, one violation was identified

(failure to maintain current controlled drawings, paragraph 4).

Licensee Maintenance Program

A major snow storm in the area of the plant at the time of the inspection

caused postponement of the inspection. Due to the limited time available

to perform this inspection only a partial inspection was performed. No

violations or deviations were identified.

3.

Licensee Maintenance Program Implementation

The NRC inspector selected five completed Station Service Request (SSR)

numbers from a list of SSRs completed on safety-related equipment. The

hard copies were checked out of Document Storage and reviewed.

Each SSR

,

was checked for authorizing sigreatures, functional testing, spare parts

control, adequacy of inspection, mechanical qualification, adequacy of

a

description portion detailing the maintenance performed and functional

testing and restoration. These SSRs were reviewed to determine if the

,

requirements of the following administrative procedures were met:

The NRC inspector interviewed the maintenance clerk who had been

scheduling the mechanical and electrical preventive maintenance (PM)

program. A master PM schedule was available and there was no backlog of

past-due preventive maintenance actions.

The PM planning and scheduling

responsibility will be turned over to the mechanical and electrical

planning group in the near future.

Licensee Instrumentation Maintenance

The NRC inspector performed an inspection of the maintenance program in

the instrument and control areas. Maintenance history, calibration

records, qualification records, procedures, work item tracking forms and

maintenance work request forms were obtained and reviewed. The following

I&C Maintenance Organization personnel were interviewed to determine their

responsibilities, functions and qualifications:

Betterment Engineering, QC inspectors and technicians.

One work activity in progress was witnessed to determine if the technician

was following the appropriate maintenance procedures. The activity was

trouble shooting and repair of PDIS-21-393, a differential pressure

instrument across the suction filter for the circulator backup bearing

water pump on SSR 86503939. The following concern was identified:

SSR 86503939 indicated that switch PDIS-21-393 was pegged high.

In order

to determine control circuit logic, the technician reviewed applicable

schematic drawings (E drawings). However, two schematics were not current.

It was later noted that the SSR was incorrect; switch PDIS-21-393 was

pegged low, as it should be, because the associated pump was not in

operation.

Failure to maintain current controlled drawings is a

violation of 10 CFR 50, Appendix B, Criterion VI and the Fort St. Vrain

approved Quality Assurance Plan (267/8609-01).

Licensee Electrical Maintenance

The NRC inspector inspected the licensee's electrical maintenance program.

Because of the sudden need to postpone the inspection, the licensee's

electrical maintenance procedures were given only a cursory review.

6.

Exit Interview

A snow storm and the lack of electrical power at the site made access to

the site impractical. An exit interview was not held.
