Cruse Vice President - Nuclear Energy Baltimore Gas and Electric Company (BGE) Calvert Cliffs Nuclear Power Plant 1650 Calvert Cliffs Parkway Lusby, MD 20657 4702 SUBJECT: NRC REGION 1 INTEGRATED INSPECTION REPORT NOS. 50-317/97-06 AND 50-318/97-06 AND NOTICE OF VIOLATION Dear Mr. Cruse: This report transmits the findings of the safety inspection conducted by NRC inspectors at the Calvert Cliffs Nuclear Power Plant from September 14,1997 to November 1,1997. The enclosed report presents the results of that inspection. At the conclusion of the inspection, these findings were discussed with Mr. Katz and others of your staff. Overall, we concluded that your facility was operated irt z. safe manner. We found that your control of the higher risk reduced inventory condition for replacement of a Unit 1 reactor coolar.t pump seal was very good. Also, good action was taken to stabilize Unit 1 following en automatic reactor trip on October 24,1997. The reactor trip response and subuquent startup were completed without complication. We noted that a performance assessment department inspector identified that a leak repair on the high pressure main turbine had been initiated without normal engineering assessment. The effort was stopped by the BGE inspector after identification that fire protection, injection pressure, and injection volume had not been assessed. We considered the efforts of the BGE performanco assessment department to be noteworthy in this case. During our review of the 11B reactor coolant pump sent replacement, we found that although access controls into high rtdiation areas had been tightened, radiation control f I activities during seal replacement were inadequate. Specifically, we identified inadequate f air sample surveying, an example uf a failure to follow special work permit requirements, and weak conduct of a radiation safety technician shift turnover. Based on the results of this inspection, the NRC has determined that violations of NRC requirements occurred. These violations are cited in the enclosed Notices of Violation (Notice) and the circumstances surrounding them are described in detail in the subject inspection eport. All of the violations were identified by the NRC, and included: 1) a failure to perform a 10 CFR 50.59 evaluation and f ailure to update the Final Safety Analysis R3 port for onsite storage of ammonium hydroxide; 2) f ailure to implement a procedure for combating the potential spill of ammonia; 3) failure to implement a procedure to control contamination from re used anti-contamination clothing: 4) failure to collect air 9712170363 97120S PDR ADOCK 05000317 ll[ll[l J ' lilt t ja i 11lJl,[[ ni n! s' y s Q PDR IE:01 U OFFICIAL RECORD COPY - = = = * * * - .v st 1
