SUBJECT: NRC INTEGRATED INSPECTION REPORT NO. 50-333/98-08

Dear Mr. Colomb:

This letter refers to the safety inspections conducted on November 22,1998, through

January 10,1999, at the James A. FitzPatrick nuclear facility. The inspections were performed

by the resident inspectors and regional specialists. The enclosed report presents the results of

those inspections.

During the inspection period you completed the refueling outage and returned the reactor to

operation. Your conduct of these activities was generally characterized by safety-conscious

l operations, sound engineering and maintenance practices, and careful radiological work

controls. However, the evolution of draining the reactor cavity without any emergency core

cooling systems (ECCS) operable, and the subsequent uncontrolled loss of 100 inches

(15,000 gallons) of reactor water level due to reliance on a level indication that was undergoing

maintenance, are examples of non-conservative planning and scheduling regarding readiness

to respond to an inadvertent reactor vessel draindown. Once our concerns with the draining

evolution were brought to your attention, you took appropriate action to stop the evolution and

restore sufficient ECCS to an operable status prior to proceeding. The NRC is continuing to

l review the events and willinform you of the results of our review.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
