SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT

NO. 50-440/97021(DRP)

Dear Mr. Myers:

On January 27,1998, the NRC completed an inspection at the Perry site. The enclosed report

presents the results of that inspection.

During this inspection period, your operations staff reacted well to abnormal plant events such as

an automatic reactor scram and leak of trichloroethylene. Notwithstanding the equipment failures

associated with these events, operators were not unnecessarily challenged during these events.

However, we are concemed about three violations of NRC requirements that were identified

during this inspection period. In the first instance, NRC inspectors identified that combustible

material had been placed in a combustible free zone in the emergency service water pump

house. In the second instance, NRC inspectors identified that a fire door was stuck open in a

diesel generator room. In the third instance, NRC inspectors identified that raceways were not

properly marked to provide a visual method to ensure proper separation of cables is maintained.

The violations are cited in the enclosed Notice of Violation and the circumstances surrounding

the violations are described in detailin the enclosed report. Please note that you are required to

respond to this latter and should follow the instructions specified in the enclosed Notice when

preparing your response. The NRC will use your response, in part, to determine whether further

enforcement PClion is necessary to ensure compliance with regulatory requirements.
