requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violation is listed below:
10 CFR 31.5(c)(3) require, in part, that the general licensee shall assure that installation, servicing, and removal from installation involving the radioactive ;
materials, its shielding or containment, are performed in accordance with the instructions provided by the labels or by a person holding a specific license !

Contrary to the above, the general licensee did not assure that removal from -
installation involving the radioactive materials, its shielding or containment, was performed in accordance with the instructions provided by the labels or by a person holding a specific license pursuant to Parts 30 and 32 of this chapter or i from an Agreement State to perform such activities. Specifically, during the weekend of November 19-20, 1994, two Ohmart thickness gauges containing approximately 1200 millicuries of krypton-85, were removed from the licensee's facility by a person not holding a specific license pursuant to 10 CFR Parts 30 and 32 or from an Agreement State to perform such activities. l This is a Severity Level IV violation (Supplement VI).
