NRC INSPECTION REPORT NO. 50-254/97021(DRP); 50 265/97021(DRP)

Dear Mr. Pearce:

This refers to the inspection conducted on September 23 through November 3,1997, at the

Quad Cities Nuclear Power Station. The purpose of the inspection was to determine whether

activities authortzed by the license were conducted safely and in accordance with

NRC requirements. During this period, routine resident inspections were performed. The

enclosed report presents the results of this inspection. Based on the inspectors' findings, overall

performance at the Quad Cities Station was safe. At the conclusion of the inspection, the

findings were discussed with those members of your staff Identified in the enclosed repoit.

Operators demonstrated good control during complex plant manipulations. Even though the

improvements made to the maintenance and planning process have decreased work raquest

backlogs, equipment problems continued to affect unit operations and availability of supporting

equipment. You identified procedures which allowed operators to bypass the primary

containment supprossion pool function during inerting or de-inerting activities, but failed to report

instances when Technical Specifications (TS) were not met in this area. You also identified

additionalinstances where TS required surveillances were missed. This is a continuation of a

problem we identified in previous inspections. You should apply appropriate management

attention to ensure surveillance testing is accomplished in the required time intervals. Additional

occurrences of inadequate procedures to implement safe shutdown actions in the event of a fire

were identified, and will be reviewed in a separate inspection report.
