MODIFICATION NO. 4 TO TASK ORDER NO. 34 ENTITLED " TECHNICAL ASSISTANCE FOR THE REVIEW OF HOLTEC INTERNATIONAL Hi-STORM SPENT FUEL STORAGE CASK" UNDER CONTRACT NO. NRC-02-95-003
Dear Mr. Bendt:
The purpose of this modification is to extend the period of performance of the subject task order from " January 31,1999" to " February 5,1999", at no additional cost or obligation to the Government.
The issuance of this modification does not change any other terms and conditions of the task order, including the ceiling amount of $96,493.00.
A summary of obligations for the subject task order, from award date through the date of this action is given below.
FY98 Obligation Amount:
$75,000.00 Cumulative Total of NRC Obligations: $75,000.00 This modification does not obligate any FY99 funds.
Please indicate your acceptance of this modification by having an official, authorized to bind your organization, execute three (3) copies of this document in the space provided and return two (2) copies to the U.S. Nuclear Regulatory Commission, ATTN: Ms. Anita Hughes, Division of Contracts and Property Management, T-712, ADM/DCPM/CMB2, Washington, D.C. 20555.
You should retain the third copy for your records.
