Dear.Mr. Thacker:

This refers to.the telephone conve'rsat'.on on'. March-2, 1994, between you and Evelyn R._ Matson of my staff, in which you notified the NRC that a polonium-210 static eliminator was apparently1 stolen.

The device, an Amersham Model ' PDM-1002H4, was leased from Amersham. This also refers.

.i to the. telephone conversation on_ March 25, 1994, in which you notified

'us that as a result of your efforts to recover the source, the soruce r4 Lwas recovered on March 25,.1994.

.Although the source 2was recovered, a.. violation of NRC requirements.s identified..;The violation involves the unauthorized transfer of "approximately 10 mil 11 curies. (370 MBq)> of polonium-210,' and is' described,
