Attachment to P-86442 On March 17, 1985, an unplanned liquid radioactive effluent release occurred and was reported to the U. S. Nuclear Regulatory Commission on Licensee Event Report 85-004 (Reference 3) pursuant to the requirements of 10CFR50.73(a)(2)(v). This event involved the release of approximately 900 gallons of liquid during the recirculation process of a liquid waste receiver.
The apparent cause of the release was leakage through an isolation valve. This liquid was discharged through redundant activity monitors which would have tripped the recirculation pumps and closed additional isolation valves upon reaching alarm conditions.
It was determined through analysis of the tank contents as well as the dilution at the cooling tower blowdown that the resultant diluted. activity at the closest unrestricted access point was far below the MPC limits. Per Licensee Event Report 85-004, procedure changes and design changes have been initiated to prevent recurrence.
