Gentlemen:

This refers to the inspection conducted by Messrs. H. D. Chaney and B. Murray

of this office during the period June 9-12, 1986, of activities authorized by

NRC Operating Licenses R-25 and R-126 for the University of Utah's AGN-201 and

TRIGA reactors respectively, and to the discussion of our findings with

Mr. R. E. Turley, and other members of your staff at the conclusion of the

inspection.

Areas examined during the inspection included reactor operations, physical

security, radiation protection, and emergency preparedness. Within these

areas, the inspection consisted of selective examination of procedures and

representative records, interviews with personnel, and observations by the

inspectors. The inspection findings are documented in the enclosed inspection

report.

During this inspection, it was found that certain of your activities were in

violation of NRC requirements and deviated from commitments made to the NRC.

The results of the inspection are documented in the enclosed inspection report

and its attachment. As discussed in the telephone conversation between Blaine

Murray of this office and G. M. Sandquist of your staff on July 2, 1986, we

have scheduled an enforcement conference in your office at 1:00 p.m. July 21,

1986, to discuss these apparent violations and deviations.

The material enclosed as an Attachment to the Appendix herewithin contains

Safeguards Information as defined by 10 CFR 73.21 and its disclosure to

unauthorized individuals is prohibited by Section 147 of the Atomic Energy Act

of 1954, as amended. Therefore, the material in the Attachment to the Appendix

will not be placed in the Public Document Room.
