This refers to the inspection conducted by K. R. Naidu of this office on June 2-6, 1986, of your facility at Olathe, Kansas and to the discussions of our findings with you and members of your staff at the conclusion of the inspection. The enclosed inspection report documents the observations made during the disassembly, on June 25, 1936, of one of the differential pres-sure switches which malfunctioned at the LaSalle County Station Unit 2.
The purpose of this inspection was to determine whether you have effectively itnplemented a quality assurance program meeting the requirements of 10 CFR 50 Appendix B with regard to the manufacture of pressure and level switches and if you have an effective procedure to report potential defects in accordance with the requirements of 10 CFR Part 21. Areas examined during the inspection and our findings are discussed in the enclosed report. Within these areas, the inspection consisted of an examination of procedures and representative records, interviews with personnel, and observations by the inspector.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of this letter and the enclosed inspection report will be placed in the NRC's Public Document Room.
