LASALLE INSPECTION REPORT 50-373/98023(DRP); 50-374/98023(DRP)

Dear Nir. Kingsley.

This refers to the inspection conducted November 21,1998, through January 5,1999, at the

LaSalle Nuclear Power Station. The purpose of the inspection was to determine whether

activities authorized by the license were conducted safely and in accordance with NRC

requirements. At the conclusion of the inspection, the findings were discussed with those

members of your staff identified in the enclosed report. No cited violations of NRC

'

requirements were identified during the course of this inspection.

Overall, performance of plant personnel was acceptable. Operators performed well during

routine activities and effectively controlled activities in a deliberate manner. The shutdown and

startup of Unit 1 for a planned outage was also effectively controlled by the operators.

However, the inspectors noted that in two instances, involving a failure of a turbine stop valve to

fast close and declaring the high pressure core spray system operable with an inoperable

tmergency power supply, operators did not practice strict compliance with Technical

~ Specification requirements. While the circumstances involved in each instance resulted in

' neither being safety significant, together these actions demonstrated the need to maintain

proper focus on Technical Specifications. In addition, system engineering support in initially

l

responding to the failure of the turbine stop valve was not thorough and did not adequately

i

i

focus on potential safety considerations.

Your staff performed maintenance activities during the inspection period in a proficient manner

and in accordance with procedures. However, two equipment problems identified during the

shutdown for a planned outage were not effectively addressed to preclude the problems from

recurring during the subsequent plant startup. These problems, involving a malfunction of the

\\ \\

thrust bearing wear detector on a turbine driven reactor feedwater pump and a feedwater

heater isolation due to level instabilities in the feedwater heaters, demonstrated the need to

continue to emphasize proper resolution of material conditicn issues.
