NRC INSPECTIC N REPORT NO. 50-440/97016(DRP) AND
NOTICE OF VIOL.ATION
Dear Mr. Myers:
On December 1,1997, the NRC completed an inspection at the Peny site. The enclosed
report presents the results of that inspection.
During the period covered by this inspection, plant activities were generally conducted well.
- Preparations for plant startup were thorough which led to an event free retum to power from
refueling outage 6. -We are concemed, however, about several operational events which
occurred over a short period of time towards the end of the outage, included in these
events was a large recirculation flow control valve packing leak which resulted from an -
inadequate tag-out for valve actuator work. The inadequate tag-out is the subject of the
violation of NRC requirements that was identified during this inspection.- This violation is of
concern because of the importance associated with implementing and controlling
procedures and instructions that can directly affect safety during work activities.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances .
_ surrounding the violation are described in detailin the enclosed report. Please note that
you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory
requirements.
