DAMAGED LNM/ CONTAINER EVENT TEXT DAMAGED URANIUM DIOXIDE POLYPAdK CONTAINER DUE TO A MINOR FIRE.
WHILE IN THE PROCELS OF SHUTTING DOWN CONVERSION OPERA 7/ IONS, AN OPERATOR NOTICED THAT A POLYPACK CONTAINER (A 6-INCH HIGH, 8-INCH DIAMETER, PLASTIC CONTAINER THAT HOLDS URANIUM DIOXIDE POWDER) ON CONVERSION LINE 2 WAS SMOLDERING IN THE FITZMILL PRODUCT HOOD AS THE POWDER WAS BEING DISCHARGED.
SHORTLY AFTER, ANOTHER OPERATOR NOTICED A SMALL FLAME UP.
THE OPERATORS USED SOME CLEANING RAGS THAT WERE IN THE VICINITY TO SMOTHER THE MINOR FIRE.
PRIOR TO THE EVENT, THE URANIUM HEXAFLUORIDE FEED MATERIAL HAD BEEN SHUT OFF BECAUSE THE LINE WAS BEING SHUT DOWN IN PREPARATIONS FOR THE LONG HOLIDAY WEEKEND.
ABOUT THE TIME THAT THE OPERATORS NOTICED THE SMOLDERING POWDER IN THE SMALL CONTAINER, THE STEAM AND HYDROGEN SUPPLIES TO THE LINE WERE SHUT OFF.
AT THIS POINT, NOTHING COULD POSSIBLY FLOW INTO THE PRODUCT HOOD WHICH CONTAINED THE SMOLDERING URANIUM DIOXIDE POWDER.
THE OPERATORS CONTINUED TO MONITOR AND EXTINGUISH THE FLAME UPS UNTIL THE POWDER COOLED OFF IN THE HOOD.
THE SMOLDERING AND MINOR FLAME UPS WERE COMPLETELY EXTINGUISHED WITHIN APPROXIMATELY 15 MINUTES.
AFTER THE MATERIAL COOLED, THE OPERATORS REMOVED IT, REPACKAGED IT INTO DIFFERENT POLYPACK CONTAINERS, t.ND CONTINUED TO SHUTDOWN THE LINE.
AN AREA APPROXIMATELY TWO INCHES WIDE WAS MELTED OUT OF THE SIDE OF THE POLYPACK CONTAINER.
AS A RESULT, SOME OF THE URANIUM DIOXIDE POWDER SPILLED OUT OF THE CONTAINER ONTO THE FLOOR OF THE HOOD.
THE HOOD WAS NOT DAMAGED AND REMAINED FUNCTIONAL DURING THIS EVENT.
THE LEVEL PROBE TO THE CONTAINER AND ALL OF THE SAFETY SYSTEMS ALSO REMAINED FUNCTIONAL.
IN ADDITION, THERE WERE NO PERSONNEL INJURIES OR EXPOSURES AS A RESULT OF THIS EVENT.
THIS EVENT ALSO INVOLVED APPROXIMATELY 25 KILOGRAMS OF URANIUM DIOXIDE POWER WITH AN ENRICHMENT OF 4.95 WEIGHT PERCENT.

THE URANIUM DIOXIDE POWDER WAS FIRST PRODUCED IN A CALCINER (A HEATED KILN)
AND THEN IT WAS SENT TO THE FITZMILL (A CRUSHER THAT GE.NDS THE PONDER TO PRODUCE UNIFORM PARTICLE SIZES).
AFTER THE MILLING AND GRINDING PROCESSES, A CHUTE WAS CONNECTED TO THE POLYPACK CONTAINER.
THE URANIUM DIOXIDE MATERIAL HAS A LARGE SURFACE AREA, AND BECAUSE THIS MATERIAL IS PYROPHORIC, IT TENDS TO OXIDIZE VERY RAPIDLY ESPECIALLY WHEN IT IS HOT AND SUBJECTED TO OXYGEN.
IT ALSO GENERATES HEAT WHEN IT IS OXIDIZED.
THEREFORE, THE LICENSEE'S INITIAL INVESTIGATION CONCLUDED THAT A POSSIBLE CAUSE OF THE MINOR FIRE COULD HAVE BEEN A FAILED SEAL WHICH PERMITTED OXYGEN TO ENTFR THE POLYPACK CONTAINING THE HOT URANIUM DIOXIDE POWDER.
THE LICENSEE PLANS TO INVESTIGATE THIS EVENT FURTHER AND TO NOTIFY THE NRC REGION 2 OFFICE NEXT WEEK.

ON 03/20/97, THE LICENSEE BECAME AWARE THAT THEY DID NOT KNOW THE LOCATION OF TWO UNIRRADIATED FUEL RODS.
ON 03/20-21/97, AN INITIAL SEARCH WAS CONDUCTED AND KNOWLEDGE OF THE LOCATION OF THE FUEL RODS WAS NOT ESTABLISHED.
ON 03/26/97, THE COLUMBIA PLANT REGULATORY AFFAIRS MANAGER AND THE PLANT MANAGER WERE NOTIFIED OF THE TWO MISSING FUEL RODS. A FORMAL INVESTIGATION TEAM WAS FORMED AND AN IN-DEPTH INVESTIGATION WAS STARTED.
ON 04/05/97, INFORMATION WAS GATHERED THAT INDICATED THAT THE FUELS RODS MAY HAVE BEEN PLACED INTO A POPULATION OF LEAD-FILLED RODS THAT WERE USED TO MAKE TWO REPLICA FUEL ASSEMBLIES FOR THE TEMELIN POWER PLANT LOCATED IN THE CZECH REPUBLIC. THIS INFORMATION WAS GATHERED FROM THE FUEL ROD COMPUTER TRACEABILITY SYSTEM.
OH 04/05-09/97, ADDITIONAL INFORMATION WAS GATHERED THAT SUPPORTED THE SCENARIO THAT THE FUEL RODS MAY HAVE GOTTEN INTO A POPULATION OF LEAD-FILLED RODS THAT WERE USED TO MAKE TWO REPLICA FUEL ASSEMBLIES FOR TEMELIN, AND THAT THE REPLICA ASSEMBLIES WERE AT THE TEMELIN SITE IN THE NEW FUEL RECEIPT AREA.
ON 04/10/97, THE CZECH REPUBLIC UTILITY CUSTOMER, CEZ, WAS NOTIFIED OF THIS SCENARIO.

THIS OCCURRED IN THE #1 PELLET LINE (IN THE PELLETING AREA OF THE PLANT)
WERE THEY HAD AN ACCUMULATION OF 4.4 WT% UO2 POWDER IN THE AMOUNT OF 165KG WHICH WAS REMOVED AND PROCESSED UNDER CONTROLLED CONDITIONS. THE SAFETY PERSONNEL WERE NOTIFIED AROUND 0630 AND THE FIRST ACTION TAKEN WAS TO REVIEW THE CRITICALITY SAFETY EVALUATION WHICH IS THE JUSTIFICATION FOR PELLET LINE #1.
BASED ON THIS INITIAL REVIEW IT APPEARED THAT THEY STILL HAD DOUBLE CONTINGENCY PROTECTION BASED ON MODERATION CONTROL AND FAVORABLE GEOMETRY.
A SUBSEQUENT REVIEW OF THE CRITICALITY SAFETY EVALUATION REVEALED THAT THEY WERE UNABLE TO VERIFY THE FAVORABLE VOLUME GEOMETRY SINCE THE ORIGINAL DOCUMENT THAT PROVIDED THE SAFETY BASIS COULD NOT BE LOCATED.
THEY DECIDED TO REANALYZE THE PARAMETERS FOR THE SYSTEM. AT 2016 THEY CONFIRMED THAT *i'EY COULD NOT RE-ESTABLISH THE SAFE VOLUME CLAIM.
CONSEQUENTLY, THEY C'*.MENCED SHUTDOWN OF THE OTHER THREE PELLET LINES SO THAT MODIFICATIONS COULD BE MADE TO THE FEEDER HOPPERS. THESE MODIFICATIONS SHOULD BE T MPLETED BEFORE DAY SHIFT.

RESPONSE TO CONCERNS i
in this Appendix, Westinghouse addreces NRC concerns regarding the need for emergency response teams to have immediate access to rellable equipment; and,_ the issue concerning adequacy of equipment maintenaace that had been identifled by Westinghouse personnel about l
eight months prior to the inspection, but not sufficiently resolved to avoki violation of Emergency l
Plar' requirements.
Westinghouse agrees that emergency response teams must have lmrnxilate access to reliable equipment.
To this end, Westinghouse hs established Inventories of emergency response equipment in several strategic locations for use by emergency reslenders, including Self.

The emergency equipmeta included the SCBA's, megaphones, and quantitles of supplies an( protective equipment, in various emergency supply cabinets, and in the emergency vehicle.

- training was covered in Fire Plan training. Therefore, no specific lessons plans were developed for lese topics.
l 11.3 IMMEDIATE ACTION TAKEN AND RESULTS ACHIEVED To address the ' radiological emergency and rblation response" traltting, a script was w and distributed to Emergency Coordinators and Emergency Brigade Leadore, with Instructions to provide this training by July 31,1994. 'the training includes (1) radiological einergency and radiadon response training,'(2) applicable radioactive matettals awareness training, and (3) applicable Site Emergency Plan and Irr.plemendng lhocedures training. The following scenarlos were included: accidental crideality (including; descriptions of historic accidental criticalities, typical dose rates from an accidental criticality, and summary 4
informadon from past criticalities at two fuel processing plants), UF6 release, uranium spill, fire, and explosion. When this training d completed, all categories in Section-7.2.&of the-Site Emergency Plan will have beca covered'.
To address the ' control of utilities and emers;ency diesels' training, a special session of tas Emergency Brigade was held June 30,1994,to cover several topics, including the " control of utilitisa and emergency diesels.'
Apppximately 47 out of 75 Emergency. Brigade members attended these sessions. 'the training included tours cf various plant. areas to review emergency shutoffs of udlities (such as electricity, hydrogen, natural gas,cheadcals, etc.) and a review of the operation, startugl, and shutdown of tre emergency diesel firs pumps. The remainder of the Emergency Bhgade members will 1:e trained during 1994.
