During the month of June, there was one unit shutdown which occurred on June 8, 1986.

Both motor-generator sets supplying power to the control rod drive systen malfunctioned. This removed the source of power to the grippers of the c3ntrol rod drive mechanisms and allowed all the i

control rods to fall into the core.

This resulted in a high negative flux rate which caused the reactor trip breakers to open. The unit returned to operation on June 9.

4 86-007-00 AA MG to the grippers of the control rod drive mechanisms and allowed 'all the control rods to fall into the core. This resulted in a high negative flux rate which caused the reactor trip breakers to open.

The 2A M set lost its field which caused the output voltage to go to zero. The output breaker of the 2B M set opened due to the actuation of a directional overcurrent relay (IRV). The reason for the loss of output of the 2A MG set has not been determined. The 28 M set should not have tripped. However, a plunger was found to be out of adjustment on an auxiliary relay of the IRV on the "C" phase of the 28 NG set.

This caused the output breaker on the 28 MG set to open when the 2A MG set stopped generating.

To prevent recurrence of this type of event, the IRVs on all MG sets at FNP have been checked and re-adjusted as required. The appropriate procedures will be revised to include the additional tasks necessary to check the auxiliary relays on the IRVs.
