SUBJECT: NOTICE OF VIOLATION AND NRC INSPECTION REPORT
NO. 50-331/98002(ORP) -
Dear Mr. Franz:
On February 4,1998, the NRC completed a routine safety inspection at your Duane Arnold
Energy Center (DAEC) facility. The enclosed report presents the results of that inspection.
During this six-week inspection period, your conduct of activities at the DAEC facility was
characterized by professional operations with appropriate focus on safety, sound maintenance
,
practices, and good teamwork among departments.

We are concemed, however, about four violations of NRC requirements that were identified. The
first three violations occurred when the limiting condition for operation of several containment
isolation valves was exceeded. This occurred over two years and there were three specific
examples identified. Also of concern is that your detailed review of this issue was not initiated
until after the inspectors communicated their concems on your compliance with Technical
Specifications on several occasions. In the fourth violation, the NRC inspectors identified that
your testing of the control building chiller la November 1997 did not meet 10 CFR 50.59
requirements.
These violations are cited in the enclosed Notice of Violation (Notice), and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that you are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
