Dear Sir:

Special Report Limiting condition for operation (LCO) 3.7.8.2 of the Trojan Technical Specifications requires a Special Report be provided if a continuous fire watch is not established within one hour when one or more of the required spray, sprinkler and/or deluge systems are inoperable.

Deluge systems 18 and 19 in the electrical penetrhtion areas were removed from service each day for one shif t from May 8,

1986 to May 26, 1986 while performing outage-related welding in these areas.

The deluge systems were returned to service for the back shifts.

On May 27, 1986 the systems were again removed from service for performance of the welding.

The welders, however, were prevented from entering the electrical penetration area due to performance of containment integrated leak rate testing.

The welders returned the burning and welding permit to the Control Room, but failed to notify the Shift Supervisor that the deluge systems should be returned to service.

A continuous fire watch was never set and the deluge systems remained inoperable until June 1, 1986.

On June 1, 1986 the deluge systems were discovered to be isolated during performance of a valve lineup verification surveillance.

The work group supervisor was counseled concerning the need to keep the Control Room informed of equipment status and 4

the need to take compensatory action when equipment operability requirements are not met.
