NRC INSPECTION REPORT NO. 50-295/97022(DRP); 50-304/97022(DRP) AND
Dear Mr. Brons:
On October 10,1997, the NRC compkted an inspection at your Zion 1 and 2 reactor facilities.
The enclosed report presents the results of that inspection.
During this 6 week inspectlan period, the laspectors noted some improvement in the operators'
implementation of the Zion Operations Department Standards. This was most evident in the
operators' response to the unexpected loss of instrument bus 213. However, the inspectors also
n;ted that the implementation of the out-of-service program remained problematic and that
operators continue to be challenged by inappropriate procedural guidance.
We are concemed about four violations of NRC requirements that were identified during this
inspection period. The first violation Irwolves three examples of deficiencies in the
implementation of the out-of-service program. The second violation pertains to the failure to
provide appmpriate guidance to the operators to test the autostart inhibit circuitry for the
OC component cooling water pump and to respond to a loss of instrument bus 213 event. The
third violation relates to the establishment of the nuclear instrumentation power range rate trip
setpoint greater than that allowed by the Technical Specification Limiting Safety System Setting.
The fourth violation involves the failure to maintain the minimum number of operable power
range rate trip channels or comply with the Technical Specification Action Statements.
These violations are cited in the enclosed Notice of Violation (Notice), and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that you are
required to respond to this letter and should follow the instructions specified in the enclosed
Notice when preparing your response. The NRC will use your response, in part, to determine
whether further enforcement action is necessary to ensure compliance with regulatory
requirements.
