Dear Dr. Patrick:

In accordance with the task order procedures of the subject contract, this letter definitizes Modification No. 5 to Task Order No.1. This effort shall be performed in accordance with the enclosed Statement of Work and the changes to the Contractor's technical proposal submitted for this modification. The effective date of this task order modification shall be January 29,1996.

This modification decreases the cost ceiling of this task order from $137,190 to $88,320.

The amount of $79,537 represents the total estimated reimbursable costs, the amount of

$2,420 reposents the cost of facility capital, and the amount of $6,363 represents the fixed fee for this task order.

Accounting Data for Modification No. 5 to Task Order No.1 is as follows:

Section 6.0, Scope of Work. paragraph B. is revised its follows:

1. The original task order envisioned that a decommissioning plan would be available for~

review, but only a remediation plan or a portion of a decommissioning plan is available.

2. The scope of the contractor's review is limited to only groundwater issues.

3.' The ' portion of the" review ~which addresses contaminated soils and structures will not be performed by the contractor.

Section 7.0, Deliverables, item 8, ' Site Status Repert' is deleted.

The contractor shall coordinate a revised schedule for the remaining deliverables with the technical monitor. However, the period of performance specified in the task order letter shall not be exceeded.
